Atherectomy system with a guide-wire.

Abstract

An atherectomy system for coring (10), ingesting and removing an obstruction material (12) from within a patient's vessel having a flexible guide-wire (140) defining voids for containing obstruction material and defining a diametrical envelope, a flexible rotary-catheter (21) slidable and rotatable over the diametrical envelope having a rotary coring means (22) at its distal end, a continuous passage defined around the flexible guide-wire by the flexible rotary-catheter for ingesting the cored obstruction material, and coupling means at the proximal end of the flexible rotary-catheter for coupling to rotating means.

    A atherectomy system for coring, ingesting and removing an obstruction material from within a patient's vessel, comprising in combination:a flexible guide-wire insertable into the vessel, a flexible rotary-catheter slidable over said flexible guide-wire, having a rotary coring means at its distal end, a continuous passage defined around said flexible guide-wire by said flexible rotary-catheter for ingesting the cored obstruction material, coupling means at a proximal end of said flexible rotary-catheter for coupling to rotating means, said flexible guide-wire having a diametrical envelope over which said flexible rotary-catheter is rotatably and slidably supported said envelope defining voids for containing the obstruction material.

    Apparatus for removing an obstruction from a vessel with an atherectomy system comprising:a flexible guide-wire for insertion into a vessel, into an obstruction, and having a diametrical envelope which contains voids for accepting the obstruction material, rotary coring means located at a distal end of an atherectomy catheter for being advanced over the flexible guide-wire, means for advancing and rotating the rotary coring means into the obstruction and coring the obstruction while the rotary coring means is rotatably supported and aligned with an artery by the diametrical envelope, and means for permitting removal of the atherectomy system containing the obstruction material out of the vessel.

    An atherectomy system for coring, ingesting and removing an ostruction from a patient's vessel, comprising in combination:a flexible guide-wire insertable into the vessel, a flexible rotary-catheter being rotatably disposed and insertable into the vessel, over said flexible guide-wire, a rotary coring means, at a distal end of said flexible rotary-catheter, for making a peripheral cut in an obstruction located in front of said rotary coring means, a rotating inner-wall formed by the inside surface of said rotary coring-means and said flexible rotary-catheter, a continuous passage defined around said flexible guide-wire by said rotating inner wall for ingesting the cored obstruction material, the relative motion between said inner-wall and said flexible guide-wire mechanically enabling the ingested obstruction material to move proximally in said continuous passage, coupling means at said proximal end of said flexible rotary-catheter for coupling to rotating means.

    An atherectomy system as in any preceding claim, wherein said flexible rotary-catheter has a higher torque carrying capacity in the vicinity of said proximal end than in the vicinity of said distal end.

    An atherectomy system as in any preceding claim, wherein said flexible rotary-catheter has a torque limiting clutch between said proximal end section to said distal end section.

    An atherectomy system as in any preceding claim having a flexible sleeve in which said flexible rotary-catheter is rotatably disposed.

    Apparatus for removing an obstruction from an vessel with an atherectomy system, comprising:means for inserting into a vessel, into an ostruction, a flexible guide-wire, means for advancing over the flexible guide-wire a rotary coring means located at a distal end of an atherectomy catheter, means for advancing and rotating the rotary coring means to the obstruction and coring the obstruction, means for mechanically enabling the cored material to move proximally in a continuous passage defined between the atherectomy catheter and the flexible guide-wire, and means for removing the atherectomy system containing the ostruction material out of the vessel.

    Apparatus for mechanically drilling through an arterial ostruction, comprising:a mechanical atherectomy system for insertion means for advancing said mechanical atherectomy system through said obstruction so as to drill a lumen having an area in the range of one half to three quarters of the area of the unobstructed arterial lumen.
