Handle for manipulating a catheter tip, catheter system and medical insertion system for inserting a self-expandable heart valve stent

Abstract

This invention is directed to a handle (70) for manipulating a catheter tip (10) of a catheter insertion system (100), with which a self-expandable heart valve stent (150) can be inserted into the body of a patient. The invention further concerns a catheter system (30) of a catheter insertion system (100) as well as a catheter insertion system (100) itself. The proposed catheter insertion system (100) is designed to enable the implantation of a heart valve prosthesis (160) attached to a heart valve stent (150) in the optimum implantation location in a sequence of events defined before the intervention. For this purpose, the catheter insertion system (100) comprises the handle (70) for manipulating the catheter tip (10) of the catheter insertion system (100), wherein the handle (70) comprises means (71, 81) that prescribe a pre-set sequence of steps such that each subsequent step is inhibited until the preceding step has been completed.

    Claims 1. A handle for manipulating a catheter tip of a medical insertion system, wherein the handle has at least a first and a second operating means, with which first and 5 second portions of the catheter tip can be manipulated so that a self-expandable stent housed in the catheter tip can be released in steps in a previously defined or definable sequence of events from the catheter tip, wherein the handle further has a first stop associated with the first operating means and a second stop associated with the first operating means, wherein the 10 first and second stop are designed to limit an overall longitudinal displacement stroke of a first portion of the catheter tip that can be effected by actuation of the first operating means;and wherein the handle has a first stop associated with the second operating means and a second stop associated with the second operating means, wherein the first 15 and second stop are designed to limit an overall longitudinal displacement stroke of the second portion of the catheter tip that can be effected by actuation of the second operating means.

    A handle in accordance with claim 1, 20 wherein the at least one operating means is designed to co-operate with the first portion of the catheter tip, and wherein the at least one second operating means is designed to co-operate with the second portion of the catheter tip.

A handle in accordance with any one of the preceding claims, wherein the handle further has a third stop associated with the first operating means, which co-operates with the first stop of the first operating means on the one side and with the second stop of the first operating means on the other side so that, when the first operating means is actuated, a stepwise longitudinal displacement of the first portion of the catheter tip, consisting of at least two individual steps, can be effected, wherein the first and third stop of the first operating means on the one side and the second and third stop of the first operating means on the other side determine the longitudinal displacement stroke of the first portion of the catheter tip that can be effected by actuation of the first operating means. A handle in accordance with any one of the preceding claims, wherein the second operating means has a second carriage guided in a second guide and functionally connected to a second slider knob, wherein the second carriage can be connected to the second portion of the catheter tip, so that when the second operating means is actuated a direct transmission of force can be realised from the second carriage to the second portion of the catheter tip. A handle in accordance with any one of the preceding claims, wherein the first operating means has a first carriage guided in a first guide and functionally connected to at least one first slider knob, wherein the first carriage can be connected to the first portion of the catheter tip, so that when the first operating means is actuated a direct transmission of force can be realised from the first carriage to the first portion of the catheter tip. A handle in accordance with claim 4 and claim 6, wherein the third stop is embodied as a stop element releasably fastened to the first guide. A handle in accordance with claim 6 or 7, wherein the first operating means has a detent spring mechanism with at least one spring element and with at least one catch formed in the first guide; wherein 8. 2008250552 09 Dec 2011 -72the first slider knob has at least one engaging element formed complementary to the at least one catch; and wherein the at least one spring element is designed and co-operates with the first slider knob on one side and with the first carriage on the other so that, in a 5 condition when the first operating means is not actuated, the first slider knob is spaced from the first carriage under spring action and that the at least one engaging element engages in the at least one catch and that, in a condition when the first operating means is actuated, the first slider knob can be pressed against the spring force of the at least one spring element in the direction of the first 10 carriage and the engagement between the at least one engaging element and the at least one catch is released. 9. A handle in accordance with claim 8, wherein the detent spring mechanism has a first catch and a least a second catch, 15 which are respectively formed complementary to the at least one engaging element and are located at positions spaced from one another in the longitudinal direction of the first guide. 10. A handle in accordance with claim 9, 20 wherein the distance between the at least one first catch and the at least one second catch defines a first longitudinal displacement stroke of the first portion of the catheter tip that can be effected with actuation of the first operating means. 11. A handle in accordance with claim 9 or 10, 25 wherein the at least one first catch and the at least one second catch are formed in a first guide surface of the first guide; and wherein the detent spring mechanism further has at least one third catch and at least one fourth catch, which are respectively formed complementary to the at least one engaging element in a second guide surface opposing the first guide 30 surface, and are located at positions spaced from the first guide in the longitudinal direction. 12. A handle in accordance with claim 11, 2008250552 09 Dec 2011 -73wherein the distance between the at least one third catch and the at least one fourth catch defines a second longitudinal displacement stroke of the first portion of the catheter tip that can be effected with actuation of the first operating means. 5 13. A handle in accordance with claim 11 or 12, wherein the at least one engaging element is formed on one of the two lateral surfaces of the first slider knob guided when the first operating means is actuated with the first and the second guide surface; and wherein the at least engaging element can be optionally brought into engagement 10 with the catches formed in the first guide surface or with the catches formed in the second guide surface. 14. A handle in accordance with any one of the claims 1 to 4, wherein the first and/or the second operating means have/has a wheel supported 15 rotatably in the handle, wherein the wheel can be connected to one of the first and second portions of the catheter tip so that a direct force transmission can be realised from the wheel to the one portion of the catheter tip. 15. A handle in accordance with claim 4 and claim 14, 20 wherein the first operating means has a wheel and wherein the third stop associated with the first operating means is formed as an engaging element, which is designed to restrict the ability of the wheel to rotate freely between the first and the second stop of the first operating means. 25 16. A handle in accordance with claim 4 and claim 14, wherein the first operating means has a wheel, and wherein the third stop associated with the first operating means is embodied with a locking element, which is designed to interrupt a direct transmission of force from the wheel to the first portion of the catheter tip, wherein the locking element is located removably 30 in the force flow between the wheel and the first portion of the catheter tip. 2008250552 09 Dec 2011 -7417. A catheter system of a medical insertion system for inserting a self-expandable heart valve stent into the body of a patient, wherein the catheter system has first and second force transmission means, which can be connected for manipulation 15 of first and second portions of the catheter tip on the one side at the proximal end portion of the catheter system with the catheter tip and on the other side at the distal end portion of the catheter system with the first and second operating means of a handle according to one of the claims 1 to 17. 20 19. A catheter system in accordance with claim 18, wherein the catheter system has a first force transmission means, which can be connected at its distal end portion with the at least one first operating means of the handle and at its proximal end portion with the first portion of the catheter tip and is designed to transmit a tensile or shear force to the first portion of the 25 catheter tip when the at least one first operating means is actuated; and wherein the catheter system further has a second force transmission means, which can be connected at its distal end portion with the at least one second operating means of the handle and at its proximal end portion with the second portion of the catheter tip and is designed to transmit a tensile or shear force to 30 the second portion of the catheter tip when the at least one second operating means is actuated. 2008250552 09 Dec 2011 -7520. A catheter system in accordance with claim 20, wherein the catheter system has a filler body formed from a filler material, which is located between which the one force transmission means and the other force 10 transmission means, in a portion of the catheter system, in which the other force transmission means is mounted in the interior of the one force transmission means. 22. A catheter system in accordance with claim 21, 15 wherein a guide channel is formed in the filler body, running in the direction of the longitudinal axis of the catheter system, through which the other force transmission means is guided. 23. A catheter system in accordance with claim 21 or 22, 20 wherein at least one fluid channel is formed, running in the direction of the longitudinal axis of the catheter system, to pass fluid from the proximal end portion of the catheter system to the distal end the portion of the catheter system and the reverse. 25 24. A catheter system in accordance with claim 23, wherein the at least one fluid channel can be connected to a first syringe adapter forming part of the handle on one side and to the catheter tip on the other. 25. A catheter system in accordance with one of the claims 20 to 24, wherein the 30 other force transmission means is in the form of a capillary tube extending from the proximal end portion to the distal end portion of the catheter system, through which a guide wire can be passed. 2008250552 09 Dec 2011 -7626. A catheter system in accordance with one of the claims 18 to 25, wherein the first force transmission means is in the form of an external catheter and can be connected at its proximal end portion to the first portion of the catheter tip in the form of a stent sheath; and 5 wherein the second force transmission means is in the form of an internal catheter, which is mounted at least in sections in the interior of the external catheter, wherein the proximal end portion of the internal catheter can be connected by means of a head section of the catheter tip to the second portion of the catheter tip embodied as a stent funnel. 27. A catheter system in accordance with claim 26 wherein the catheter system has a sheath body formed from a filler material, which is located in the portion of the catheter system in which the internal catheter is mounted in the interior of the external catheter between the internal 15 catheter and the external catheter, wherein the filler body can be fixed with its distal end portion rigidly to an anchorage of the handle and can be fastened with its proximal end portion to a stent holder of the catheter tip. 28. A catheter system in accordance with claim 26 or 27 20 wherein the internal catheter can be rotated relative to the external catheter about the longitudinal axis of the catheter system, and wherein the filler body can be fastened with its proximal end portion to the stent holder, so that the filler body can be rotated relative to the stent holder about the longitudinal axis of the catheter system. 29. A catheter system in accordance with one of the claims 26 to 28, wherein at least a proximal portion of the catheter system is in the form of a flexural link section, which is elastically deformable; and wherein the catheter system further has a control wire, which runs through a 30 channel formed in the filler body from the distal end portion of the catheter system to the proximal end of the sheath body and can be fixed with its proximal end portion to a stent holder of the catheter tip and can be connected with its distal end portion to a third operating means of the handle. 2008250552 09 Dec 2011 - 7730. A catheter system in accordance with one of the claims 18 to 25, wherein the second force transmission means is in the form of an external catheter, wherein the catheter system further has a cannula tube, which is 5 mounted at least in sections in the interior of the external catheter, and wherein the first force transmission means is in the form of an internal catheter, which is mounted at least in sections in the interior of the cannula tube. 31. A catheter system in accordance with claim 30, 10 wherein the cannula tube is designed so that it can be fixed rigidly at its distal end portion to an anchorage of the handle and rigidly at its proximal end portion to a stent holder of the catheter tip. 32. A catheter system in accordance with claim 30 or 31, 15 wherein the external catheter can be connected at its proximal end to the second portion of the catheter tip embodied as a stent funnel and can be connected to the internal catheter by means of a headpiece of the catheter tip with the first portion of the catheter embodied as stent sheath. 20 33. A catheter system in accordance with one of the claims 30 to 32, wherein the proximal end portion of the internal catheter exhibits reduced rigidity in comparison to the distal end portion of the internal catheter. 34. An insertion system for inserting a self-expandable heart valve stent into the 25 body of a patient, wherein the insertion system has the following:a catheter system according to one of the claims 18 to 33, by means of which the stent can be introduced into the body of a patient in its folded-together state;a catheter tip provided at the proximal end portion of the catheter system, in which the stent can be received;and 2008250552 09 Dec 2011 -78a handle provided at the distal end of the catheter system according to one of the claims 1 to 17, with which the first and second portions of the catheter tip can be manipulated. 5 35. An insertion system in accordance with claim 34, wherein the catheter tip has a stent holder for releasably fixing of at least one fastening portion of the stent in the catheter tip and a first portion connected to the first force transmission means of the handle for mounting of first functional components of the stent and a second portion connected to the second force 10 transmission means of the handle for mounting of second functional components of the stent. 36. An insertion system in accordance with claim 35, wherein the first portion is located at the proximal end portion of the catheter tip 15 and the second portion of the catheter tip is located between the first portion and the handle, wherein the first portion can be moved away from the handle in the longitudinal direction of the catheter tip relative to the stent holder when the first operating means are actuated;and wherein the second portion of the catheter tip can be moved towards the handle 20 in the longitudinal direction of the catheter tip relative to the stent holder when the second operating means are actuated. 37. An insertion system in accordance with claim 35, wherein the second portion of the catheter tip is located at the proximal end 25 portion of the catheter tip and the first portion of the catheter tip is located between the second portion of the catheter tip and the handle;wherein the second portion of the catheter tip can be moved away from the handle in the longitudinal direction of the catheter tip relative to the stent holder when the second operating means is actuated;and 30 wherein the first portion of the catheter tip can be moved towards the handle in the longitudinal direction of the catheter tip relative to the stent holder when the first operating means is actuated. 2008250552 09 Dec 2011 - 7938. An insertion system in accordance with one of the claims 35 to 39, wherein both the first portion and the second portion of the catheter tip 15 respectively implemented as sleeve-like elements;and wherein the inside diameter of the first portion of the catheter tip is larger than the outside diameter of the second portion of the catheter tip, so that the first portion of the catheter tip is designed to accommodate, additional to the second functional components of the stent, the second portion of the catheter tip with the 20 first functional components of the stent accommodated therein. 41. A medical device for treatment of a heart valve defect, in particular a heart valve failure or a heart valve stenosis in a patient, wherein the medical device has an insertion system in accordance with claims 34 to 40 and a self-expandable heart 25 valve stent accommodated in the catheter tip. 42.
