Abstract
The invention relates to an implantable arrangement for stimulating a human or animal heart, comprising a processor (340), a memory unit (350), a stimulation unit (320) for stimulating a His bundle of a human or animal heart and a detection unit (330) for Detect an electrical signal from the same heart. - 2Implantierbare Anordnung nach Anspruch 1, dadurch gekennzeichnet, dass die erste Klasse und die zweite Klasse sowie eine optionale weitere Klasse mit Ausnahme der dritten Klasse ausgewählt sind aus der Gruppe bestehend aus einer Klasse, die einen Erregungszustand betrifft, für den ausschließlich eine Stimulation des His-Bündels des stimulierten Herzens erfolgt ist, einer Klasse, die einen Erregungszustand betrifft, für den sowohl eine Stimulation des His-Bündels des stimulierten Herzens als auch eine Stimulation des ventrikulären Myokards erfolgt ist, einer Klasse, die einen Erregungszustand betrifft, für den ausschließlich eine Stimulation des ventrikulären Myokards des stimulierten Herzens erfolgt ist, einer Klasse, die einen Erregungszustand betrifft, bei dem eine His-Bündel-Stimulation und eine Stimulation linksventrikulärer Leitungsbahnen erfolgt ist, und einer Klasse, die einen Erregungszustand betrifft, bei dem eine His-Bündel-Stimulation ohne Stimulation linksventrikulärer Leitungsbahnen erfolgt ist.
- 3Implantierbare Anordnung nach Anspruch 1 oder 2, dadurch gekennzeichnet, dass das Programm den Prozessor (340) dazu veranlasst, jede mittels der Stimulationseinheit (320) ausgeführte kardiale Stimulation zu klassifizieren.
- 4Implantierbare Anordnung nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass die Anordnung (300) einen einzigen Elektrodenanschluss für eine Stimulations- und Detektionselektrode aufweist, wobei die Detektionseinheit (330) sowohl zur Detektion einer intrinsischen Erregung eines Atriums als auch zur Detektion His-Bündel-spezifischer kardialer Signale des zu stimulierenden Herzens vorgesehen und eingerichtet ist.
- 5Implantierbare Anordnung nach einem der Ansprüche 1 bis 3, dadurch gekennzeichnet, dass die Anordnung (300) mindestens zwei Elektrodenanschlüsse aufweist, die zum Anschluss mindestens zweier Stimulations- und Detektionselektroden dienen.
- 6Implantierbare Anordnung nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass das Programm den Prozessor (340) dazu veranlasst, mindestens zwei Reizschwellenmesswerte zum selben Zeitpunkt zu bestimmen und in einem Messwertspeicher abzuspeichern, wobei ein erster Reizschwellenmesswert eine Reizschwelle eines ersten Erregungszustands des stimulierten Herzens und ein zweiter Reizschwellenmesswert eine Reizschwelle eines zweiten Erregungszustands des stimulierten Herzens angibt.
- 7Implantierbare Anordnung nach Anspruch 6, dadurch gekennzeichnet, dass der erste Erregungszustand und der zweite Erregungszustand ausgewählt sind aus einem Erregungszustand, für den ausschließlich eine Stimulation des His-Bündels des stimulierten Herzens erfolgt ist, einem Erregungszustand, für den sowohl eine Stimulation des His-Bündels des stimulierten Herzens als auch eine Stimulation des ventrikulären Myokards erfolgt ist, und einem Erregungszustand für den ausschließlich eine Stimulation des ventrikulären Myokards des stimulierten Herzens erfolgt ist.
- 8Implantierbare Anordnung nach einem der Ansprüche 6 und 7, dadurch gekennzeichnet, dass das Programm den Prozessor (340) dazu veranlasst, einen zeitlichen Verlauf der mindestens zwei Reizschwellenmesswerte abzuspeichern.
- 9Implantierbare Anordnung nach einem der Ansprüche 6 bis 8, dadurch gekennzeichnet, dass die Anordnung (300) die Möglichkeit eröffnet, für jeden der mindestens zwei Reizschwellenmesswerte einen Warnschwellenwert festzulegen, wobei das Programm den Prozessor (340) dazu veranlasst, einen Warnhinweis auszugeben, wenn zumindest einer der Warnschwellenwerte überschritten wird.
- 10Implantierbare Anordnung nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass das Programm den Prozessor (340) dazu veranlasst, vor der Ermittlung des Erregungszustands eine Signalqualitätsüberprüfung durchzuführen, um zu ermitteln, ob die zur Ermittlung des Erregungszustands vorgesehenen Signale zur Auswertung geeignet sind.
- 11Implantierbare Anordnung nach Anspruch 10, dadurch gekennzeichnet, dass das Programm den Prozessor (340) dazu veranlasst, in Abhängigkeit der durchgeführten Signalqualitätsüberprüfung automatisch einen Signalverarbeitungsparameter der Anordnung anzupassen.
- 12Implantierbare Anordnung nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass das Programm den Prozessor (340) dazu veranlasst, ein morphologisches Analysekriterium aus einem abgeleiteten Elektrokardiogramm oder ein morphologisches Analysekriterium aus einem mittels einer der Stimulationseinheit (330) zugehörigen Elektrode gewonnenen Elektrokardiogramm zur Klassifikation des Erregungszustands heranzuziehen.
- 13Implantierbare Anordnung nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass das Programm den Prozessor (340) dazu veranlasst, einen zeitlichen Versatz zwischen der Ausführung der kardialen Stimulation und der Detektion des kardialen elektrischen Signals zur Klassifikation des Erregungszustands heranzuziehen.
- 14Computerprogrammprodukt mit einem computerlesbaren Code, der einen Prozessor (340) dazu veranlasst, die folgenden Schritte auszuführen, wenn er auf dem Prozessor (340) ausgeführt wird:a) Ausführen einer kardialen Stimulation mittels einer Stimulationseinheit (320), b) Detektieren eines kardialen elektrischen Signals, das durch eine kardiale Erregung infolge der zuvor durchgeführten kardialen Stimulation erzeugt wurde, mittels einer Detektionseinheit (330), c) Ermittlung eines Erregungszustands eines durch die kardiale Stimulation stimulierten Herzens anhand des elektrischen Signals, d) Klassifikation des Erregungszustands in eine von mindestens drei Klassen, wobei die mindestens drei Klassen eine erste Klasse für einen ersten Erregungszustand, eine zweite Klasse für einen von dem ersten Erregungszustand unterschiedlichen zweiten Erregungszustand und eine dritte Klasse für eine erfolglose Stimulation ohne detektierbaren Erregungszustand umfassen, e) automatische Anpassung mindestens eines Steuerparameters einer implantierbaren Anordnung (300) zur Stimulation eines menschlichen oder tierischen Herzens in Abhängigkeit der durchgeführten Klassifikation, wobei der Steuerparameter ausgewählt ist aus der Gruppe bestehend aus einer His-Bündel-Stimulationsenergie, einem His-Bündel-Stimulationsvektor, einer Betriebsart der Anordnung (300), mindestens einem Parameter eines Zeitgebers der Anordnung (300) und einem für eine kardiale Resynchronisationstherapie geeigneten Stimulationssteuerparameter.
