Percutaneous entry system

Abstract

Apparatus to achieve percutaneous entry to a blood vessel includes several coacting combinations of coacting instruments, some capable of independent action, as well as enabling clear visualisation of the operative area, precise location of the operative site, capture and immobilization of the vessel, and guidance in the positioning and insertion of the puncturing needle. Additionally, apparatus is provided for the placement of a sponge directly on the entry wound in the vessel to reduce bleeding. A dissecting-retracting tool is described that creates a skin-to-vessel channel. The vessel is inspected through the spread fingers in the tool. A tubular access conduit, mounted on an obturator/carrier, is also described.

    A spreading instrument comprising:first and second arms having first and second handles extending proximally therefrom;pivot means coupling the said first and second arms and permitting said arms to move relative to one another upon movement of said handles;first and second fingers extending distally from the distal ends of said first and second arms;an inner surface on each of said fingers which confronts and contacts the surface on the other finger when said fingers abut one another;a rounded groove in the inner surface of each of said fingers extending along the length of the finger, each said groove having a constant radius of curvature and having a depth and arc length which decrease along its length distally;said spreading instrument being so configured that said fingers can be moved from an abutting position in which the said confronting surfaces are in contact to a spread position in which the said fingers are angled away from one another and in which the said grooves coact to partially define a cylindrical passage configured to receive a tubular structure.

    An access conduit for use in percutaneous access to blood vessels or other target organs in a human body comprising:an elongate hollow tubular structure having a proximal end, a distal end angled from 20 degrees to 80 degrees relative to the longitudinal axis of said tubular structure to create an angled opening at said distal end, and having a longitudinal groove along the inner surface, parallel to the longitudinal axis of said hollow tubular structure, extending to said angled opening.

    The access conduit of any of claims 8-9 wherein said longitudinal groove when viewed in cross-section has a curvilinear arc shape.

    The access conduit of any of claims 8-10 wherein said longitudinal groove is sized to interface with the distal tip of an elongate instrument, preferably a needle.

    A system for inserting a tubular member into the human body or into a body structure comprising:a spreading instrument according to any of claims 1 to 7;an access conduit according to any of claims 8 to 12;wherein said instrument fingers are spread to create an open cylindrical channel within said fingers, and said access conduit is inserted therein.
