Abstract
TLSO/LSO SPINE BRACE ABSTRACT The disclosure provides a brace for supporting a spine of a patient. The brace includes an anterior panel, a posterior panel, lateral panels bridging between the anterior panel and the posterior panel, and an adjustable belt tightenable around the anterior panel, the brace selectively configurable to include either lateral panels attachable to the posterior, a posterior thoracic extension attachable to the posterior panel or a dorsal lumbar extension attachable to the posterior panel. Full description of the background of the selection
The present disclosure relates to spine braces. More specifically, the disclosure relates to an improvement of the lumbar-sacral orthosis and the thoracic-lumbar-sacral orthosis to support a patient's back and spine allowing for multiple configurations 5 using combinations of the same basic components.
US20190070033 discloses a lumbosacral orthosis attachment for adapting an orthosis to operate on lumbosacral orthosis by attaching ligaments to the orthosis.
US6478759 reveals a lumbosacral thoracic support made of flexible materials designed to fit patients of all shapes and sizes. The support consists of a front support and a back support, both 10 made of hook and loop material, connected by two sets of connecting straps.
US20110105971 discloses a thoracic lumbosacral orthosis that includes an upper body support for use with a trunk orthosis. The upper body support has a front assembly that includes a front plate attached to the trunk orthosis, a sternal assembly attached to the front plate, and a thoracic assembly attached to the sternal assembly.
<p dir="rtl">15 US20130184625 discloses an elongate spinal frame having a rectangular stem frame defining the upper, middle, and lower portions, and a lumbar assembly including a lumbar bundle attached to the lower portion of the spinal frame.</p>
There is a desire to improve the design of spinal supports. In particular, improvements in spinal supports and improved configurable structures for use as lumbar splints are desirable.

The detection helpfully provides a brace system that can be configured into multiple brace configurations to support the patient's spine.
In one aspect, the bracket according to the disclosure includes an anterior panel; a posterior panel, a pair of side panels connecting the anterior panel and the posterior panel; and a retractable fastener.
<p dir="rtl">5 Adjustable to attach around the front, back and side panels. The brace is selectively configurable to include a pair of side panels attached to the back, a posterior thoracic extension that can be attached to the back panel or a dorsal lumbar extension that can be attached to the back panel.</p>
In another aspect, the support system according to disclosure comprises removable side panels, adjustable waist belts 10, a removable front panel, a sternal padding that can be attached to the front panel and adjusted in height relative to the front panel, a back panel with a removable posterior thoracic extension, and a removable dorsal lumbar extension.

Detailed description:
<p dir="rtl">10 Referring to the drawings, the disclosure provides a spinal support system 10 for the construction of an optimizer and is configured to provide multiple spinal support configurations. In particular, the spinal supports provided by the spinal support system 10 are configurable for use as a lumbar-sacral splint (LSO) and two thoracolumbar-sacral orthosis (TLSO) configurations, one with a sternal pad (TLSO-S) and the other with a lumbar dorsal extension (TLSO-D).</p>
<p dir="rtl">15 The spinal braces are specifically designed to provide support and stabilization in the sagittal, coronal, and/or transverse planes to aid in healing and recovery from post-surgical fusion, post-surgical laminectomy, post-surgical discectomy, compression fractures, degenerative disc disease, osteoporosis, chronic low back pain, spondylolisthesis, spondylolysis, spinal stenosis, osteoporosis, and facet syndrome.</p>
<p dir="rtl">20 With initial reference to Figure 1, the spinal brace system 10 includes components that can be selectively used in different combinations to provide the desired brace configuration that is most optimal for the patient’s injury. For example, as shown, the spinal brace system 10 includes as selectable components: shearable side panels 20, an adjustable brace 30, a front panel 40 having a sternal pad kit (SPK) 50, and a back panel 60 having an extension</p>

The side panels 20, back panel 60 and back thoracic extension 70 are made of injection molded plastic and compressed foam. Therefore, in conjunction with the adjustable brace 30, when the brace is applied around the patient according to the detection, it will compress around the patient's midsection to provide stability to the spine.
<p dir="rtl">5 By selectively utilizing components 20-80, the spine support system 10 is convertible to three different spine support configurations. The spine support configuration 10 TLSO-S is illustrated in Figure 2 and Figure 8A-8D. The spine support configuration 10 LSO-B is illustrated in Figure 3 and Figures 9D and 10A-10C. The spine support configuration 10 TLSO-D is illustrated in Figure 4 and Figure 11B and 12A-12B.</p>
<p dir="rtl">10 As discussed in detail below, Figures 9a-9d illustrate the conversion from a TLSO-S configuration to an LSO configuration. Figures 11a-11b illustrate the conversion from an LSO configuration to a TLSO-D configuration.</p>
Referring to Figure 1, the trimmable side panels 20 have panel sections 20a, 20b, 20c, and 20d that are connected by cutting guides 22. The cutting guides 22 are configured to be precise layouts between the panel sections that are easy to cut and follow during cutting. Hook/loop material 15 24 may be provided on the proximal ends of the panels 20 for releasable attachment of the side panels 20 to the panel.

The brace engagement members 36 are engaged with the brace to maintain the desired length of the brace, and are disengaged from the brace to allow adjustment of the brace length. The belt 30 also has belt ends 38. The ends 38 may be in a fixed position or may be removable and adjustable such as hook/loop materials at the ends of the belts 30. By positioning the ends 38 as desired, the belt 30 may be custom-fitted to the desired length. Once the desired belt length is determined, the desired tension of the belt 30 is achieved using tensioners.
<p dir="rtl">5 Cable 32. Also, an extended section may be used to span the ends 38 if the patient's circumference cannot position the ends 38 to overlap. The extended section may be removable to the ends 38 or the sling 30 as a hook/loop material.</p>
Referring to Figures 5A-5D, the front panel 40 has an integrated adjustable bracket 42 for adjustably mounting the shear gasket assembly 50 to the front panel 40. The bracket includes
<p dir="rtl">10 42 on fixed levers 44 practically connected to a retaining arm 46 having pins 46a and located within a channel 48.</p>
A front panel cover 40A is provided by a soft loop material attached to the front panel 40 using a hook material. The cover 40A provides a soft surface to which the hook material attached to the retainer 30 can be adjusted.
Referring to Figures 1 and 8D, the shear pad kit 50 has a shear plate 52 having 15 pads 52A supported by a tuning shaft 54 having tuning holes 54A. The shaft 54 is received in channel 48

Referring to Figures 5A-5D, to insert and adjust the height of the shear insert 52, the user will press the retaining levers 44 and this movement will move the retaining arm 46 and the pins 46A out of position.
<p dir="rtl">20 The advantage of this design is that the fixed levers 44 will drop or stop moving. Thus, the barrier arm 46 can only move backwards so far, and will not enter into permanent deformation. When the levers 44 are released, the pin 46A will be fully inserted through the holes 54A, making failure less likely.</p>
As shown in Figures 1, 6A, 7B, and 13A, the backplate 60 is a solid plate.
<p dir="rtl">25 It can be positioned at the back of the wearer’s brace adjacent to the wearer’s spine. The backplate 60 includes a flat central apex 62 configured to receive the posterior thoracic extension 70 in a fixable manner. The backplate 60 includes slots 60a, 60b, and 60c stacked along opposite sides thereof. As illustrated below, slots 60a-60c provide fixation points</p>

For the adjustable fixation straps attached to the lumbar dorsal extension 80. Similarly, the shoulder straps 56 of the sternal padding assembly 50 can be adjusted in slots 60A-60C. Referring to Figures 6A-6C, the thoracic dorsal extension 70 has a padded surface 70A, central sections 72 and rectangular clips facing sideways in opposite directions. The central sections 72 5 and rectangular clips 74 cooperate with the flat top 62 to provide a releasable fixation of the thoracic extension.

The lumbar dorsal extension 80 creates a high degree of thoracic spine stabilization. Referring to Figures 7A-7C, the extension 80 includes a tether arm 82 that is attached to the back of the backplate 60 as a hook/loop material. The lumbar dorsal extension 80 also includes straps 84 that can be 10 attached to any of the slots 60A-60C that serve as anchor points for the straps 84. The adjustable nature of the anchor points 60A-60C provides the clinician with the ability to optimize the amount of motion restriction, the comfort of the brace for the end user, and the ease of donning and doffing. The lumbar dorsal extension 80 also includes an internal rigid support 86 , preferably of plastic.
Figures 8A-8D illustrate the components of the strut system 10 configured to provide the column strut configuration.
<p dir="rtl">15 The TLSO-S vertebral brace configuration 10A, which includes the sternal insert assembly 50, the anterior plate 40, the posterior plate 60, the posterior thoracic extension 70, and the side plates 20. The brace configuration can be adjusted to accommodate the patient’s progress during recovery. For example, the sternal insert assembly 52 can be removed from the TLSO-S vertebral brace configuration 10A to give the patient a greater range of motion. Figures 9A-9D illustrate a conversion from the TLSO-S vertebral brace configuration 10A to the sternal brace configuration</p>
<p dir="rtl">20 The 10 LSO spinal brace configuration is shown in Figures 10A-10C. As shown, the conversion steps include removing the sternal insert assembly 50 and the posterior thoracic extension 70, followed by removing the side plates 20 and the anterior plate 40. Figures 10A-10C illustrate the 10 LSO spinal brace configuration. As shown, only the 30 and the 60 are included.</p>
Figures 11A-11B illustrate a conversion from a 10 LSO spine brace configuration to a

Thus, the stent system allows for multiple configurations and each of these configurations can be used as a single SKU. For example, it has been found that the 10 stent system allows for the use of only three different SKUs of effective configurations to treat more than 90 percent of patient cases. For example, the 10 LSO spinal stent configuration can be chosen for most conditions.
