Robotic surgical system utilizing a cutting bur for bone penetration and cannulation

Abstract

A surgical system (10) configured for manipulation of a bone structure comprising a first cortical region (CR1), a second cortical region (CR2) and a cancellous bone region (CBR) between the first and second cortical regions (CR1, CR2) is described. The surgical system (10) comprises a surgical tool (20) comprising a cutting bur (24) rotatable about a cutting axis (CA), a robotic manipulator (14) comprising a plurality of links (18) and joints (J) and being configured to support and move the surgical tool (20), wherein one of the plurality of links (18) is a distal link and the surgical tool (20) is coupled to the distal link, a control interface (80) coupled to the surgical tool (20) and comprising tactile controls (82, 84) configured to enable user control of a feed rate of the surgical tool (20), one or more controllers (30) being configured to control movement of one or more of the joints (J) of the robotic manipulator (14) and to control operation of the surgical tool (20). The one or more controllers (30) are configured to detect the cutting bur (24) being located off a target axis (TA) associated with the bone structure, control the robotic manipulator (14) to advance the cutting bur (24) according to a first feed rate to align the cutting axis (CA) to the target axis (TA), detect the cutting bur (24) being located on the target axis (TA), and control the robotic manipulator (14) to advance the cutting bur (24) along the target axis (TA) according to a second feed rate. One of the first feed rate and second feed rate is defined and commanded by the one or more controllers (30) based on a surgical plan, wherein the other one of the first feed rate and second feed rate is obtained from user interaction with the tactile controls (82, 84) of the control interface (80).

    A surgical system (10) configured for manipulation of a bone structure comprising a first cortical region (CR1), a second cortical region (CR2) and a cancellous bone region (CBR) between the first and second cortical regions (CR1, CR2), the surgical system (10) comprising:a surgical tool (20) comprising a cutting bur (24) rotatable about a cutting axis (CA);a robotic manipulator (14) comprising a plurality of links (18) and joints (J) and being configured to support and move the surgical tool (20), wherein one of the plurality of links (18) is a distal link and the surgical tool (20) is coupled to the distal link;a control interface (80) coupled to the surgical tool (20) and comprising tactile controls (82, 84) configured to enable user control of a feed rate of the surgical tool (20);one or more controllers (30) being configured to control movement of one or more of the joints (J) of the robotic manipulator (14) and to control operation of the surgical tool (20), the one or more controllers (30) being configured to: detect the cutting bur (24) being located off a target axis (TA) associated with the bone structure;control the robotic manipulator (14) to advance the cutting bur (24) according to a first feed rate to align the cutting axis (CA) to the target axis (TA);detect the cutting bur (24) being located on the target axis (TA);and control the robotic manipulator (14) to advance the cutting bur (24) along the target axis (TA) according to a second feed rate;wherein one of the first feed rate and second feed rate is defined and commanded by the one or more controllers (30) based on a surgical plan and wherein the other one of the first feed rate and second feed rate is obtained from user interaction with the tactile controls (82, 84) of the control interface (80).

    The surgical system (10) of any one of claims 1 to 4, comprising one or more sensors (70) configured to sense forces applied to the cutting bur (24) by the cancellous bone region (CBR), wherein, to advance the cutting bur (24) through the cancellous bone region (CBR), the one or more controllers (30) are configured to compare the sensed forces to a predetermined force threshold defined to prevent displacement of the bone structure from force applied by the cutting bur (24).

    The surgical system (10) of any one of claims 7 to 8, wherein, in response to detecting the transition between the first cortical region (CR1) and the cancellous bone region (CBR), the one or more controllers (30) are configured to control the robotic manipulator (14) to advance the cutting bur (24) according to the second feed rate.

    The surgical system (10) of any preceding claim, wherein the one or more controllers (30) are configured to:operate the robotic manipulator (14) in a semi-autonomous mode to enable movement of the surgical tool (20) along the target axis (TA);and operate the robotic manipulator (14) in a manual mode to advance the cutting bur (24) along the target axis (TA).

    The surgical system (10) of any preceding claim, wherein the one or more controllers (30) are configured to:define a first length of the target axis (TA);define a second length of the target axis (TA);control the robotic manipulator (14) to advance the surgical tool (20) along the first length of the target axis (TA) in a semi-autonomous mode;and control the robotic manipulator (14) to advance the surgical tool (20) along the second length of the target axis (TA) in a manual mode.

    The surgical system (10) of any preceding claim, wherein the one or more controllers (30) are configured to:define a first length of the target axis (TA);define a second length of the target axis (TA);control the robotic manipulator (14) to advance the surgical tool (20) along the first length of the target axis (TA) according to a non-cutting mode wherein the surgical tool (20) is not actuated;and control the robotic manipulator (14) to move the surgical tool (20) along the second length of the target axis (TA) according to a cutting mode, wherein, in the cutting mode, the surgical tool (20) is actively actuated to remove material from the bone structure.

    The surgical system (10) of any preceding claim, wherein the control interface (80) is configured such that the operator can directly grasp the control interface (80) by hand.

    The surgical system (10) of any preceding claim, wherein the tactile controls (82, 84) of the control interface (80) comprises one or more buttons (84), which when depressed, signal the one or more controllers (30) to advance the surgical tool (20) according to one of the first feed rate and second feed rate.

    The surgical system (10) of any preceding claim, wherein the tactile controls (82, 84) of the control interface (80) comprises one or more buttons (84), which when depressed, signal the one or more controllers (30) to increase or decrease one of the first feed rate and second feed rate.
