Annuloplasty systems and locking tools therefor

Abstract

Apparatus is provided including an implantable annuloplasty structure including a primary body portion and a contracting member. A contracting-member-fastener can surround the contracting member and include a clamping structure that is biased toward assuming a closed state to clamp onto the contracting member passed therethrough. A stop can be removably coupled to the fastener and maintain the fastener in an open state. A contracting-member-uptake tool can include static and dynamic cutting elements and graspers configured to pull stop proximally such that the stop pushes against and moves the dynamic cutting element with respect to the static cutting element in order to facilitate severing of the contracting member. Other applications are also provided.

    A system for use at a heart of a subject, the system comprising:an implantable annuloplasty structure (222;1522) comprising: a tubular body portion (26);and a contracting member (226) having (1) a first portion extending along a longitudinal length of the tubular body portion of the annuloplasty structure, and (2) a second portion extending away from the tubular body portion of the annuloplasty structure;a housing (1430;1530) positionable against the annuloplasty structure;a contracting-member-fastener (1460;1560) disposed at least in part within the housing, the contracting-member-fastener comprising a clamping structure that (a) is biased toward assuming a closed state, in the closed state, the clamping structure is configured to clamp onto the contracting member passed therethrough so that the contracting member is prevented from sliding through the fastener, and (b) can be flexed to an open state through which the contracting member can move;a stop (1470;1570) disposed within the fastener and configured to maintain the contracting-member-fastener in the open state;and a multi-component system (10;1500), configured to anchor the annuloplasty structure along an annulus of a valve of the heart, the multi-component system comprising: a contracting-member-uptake tool (1600), configured to apply tension to the contracting member while the housing is disposed against the annuloplasty structure, an inner tube (1504) that has a distal end that is shaped so as to define a fastener-ejector, and configured such that, while engaged with the stop, movement of the fastener-ejector removes the stop from the fastener and converts the clamping structure from the open state to the closed state to clamp onto the contracting member passed therethrough.

    The system according to any one of the preceding claims, wherein the fastener comprises a deformable element having a slanted state and a straight state, wherein the stop is configured to maintain the fastener in the slanted state, and wherein, upon removal of the stop, the fastener is configured to transition to the straight state and clamp the contracting member between the fastener and a surface of the housing.

    The system according to any one of the preceding claims, wherein the contracting member (226) extends through the housing (1530) and through the stop (1570) that is disposed within an opening of the contracting member-fastener (1460).

    The system according to any one of the preceding claims, wherein the multi-component system (10;1500) further comprises a contracting-member-severing tool (1502) which is configured such that severing of the contracting member (226) running through the tool (1502) is possible only once the contracting member has been locked in place by the fastener (1460;1560) coupled thereto.

    The system according to any one of the preceding claims, wherein the contracting-member-uptake tool comprises a handle portion (1620) that comprises a shaft (1634) and a knob (1630), the contracting-member-uptake tool being configured to apply the tension to the contracting member by rotation of the knob sliding the shaft proximally, the proximal sliding of the shaft applying a proximally-directed force to the contracting member.
