    Claims of equivalent WO 2020219392 A2 CLAIMS1. A system for augmenting a vertebral body, said system comprising:an access cannula comprising a cannula hub, and a cannula shaft extending from said cannula hub with said cannula shaft comprising a distal end positionable within the vertebral body and defining a lumen along a longitudinal axis;an introducer device comprising: an actuator configured to receive an input from a user;a shaft comprising a rigid proximal portion coupled to said actuator and defining a proximal end of said shaft, and a flexible distal portion, wherein a length of said shaft between said proximal end and a distal end is sufficient for said shaft to extend through and be operable beyond said distal end of said access cannula, wherein said flexible distal portion comprises a pre-set curve in an unconstrained state;a pulling element coupled to said actuator and to said shaft at or near said distal end with said pulling element extending along at least a portion of said pre-set curve, wherein tension on said pulling element is configured to be increased in response to the input to provided to said actuator to move said pre-set curve from said unconstrained state to a constrained state in which said flexible distal portion at least partially straightens, and wherein the tension on said pulling element is configured to be reduced to facilitate said pre-set curve moving from said constrained state to said unconstrained state to position said distal end of said shaft within the vertebral body at a target site that is offset from said longitudinal axis;and a flexible sheath at least partially overlying said shaft with said flexible sheath comprising a distal end positionable near said distal end of said shaft such that said flexible sheath is configured to extend through and be operable beyond said distal end of said access cannula with a distal portion of said flexible sheath conforming to said flexible distal portion as said pre-set curve moves between said constrained state and said unconstrained state, wherein said introducer device is removable from said flexible sheath with said distal end of said flexible sheath remaining at the target site offset from said longitudinal axis.

    The system of any one of claims 1 and 2, further comprising an expandable member assembly comprising a balloon hub adapted to be coupled to a fluid source, a balloon tube extending from said balloon hub and sized to be slidably inserted within said flexible sheath, and a balloon coupled to a distal end of said balloon tube and configured to be inflated with fluid from the fluid source to displace cancellous bone within the vertebral body, wherein said tube comprises a length such that said balloon is positioned proximate said distal end of said flexible sheath when said balloon tube is slidably inserted within said flexible sheath.

    The system of any one of claims 1-4, wherein said cannula hub further comprises a shaft hub rigidly coupled to said cannula shaft for and providing a datum, and a tuning hub movably coupled to said shaft hub and comprising an interference surface, wherein movement of said tuning hub relative to said shaft hub facilitates selective adjustment of the axial position of said interference surface relative to the datum.

    The system of any one of claims 1-6, further comprising at least two radiopaque markers disposed on said distal portion of said flexible sheath and spaced apart from one another such that, as said distal portion is curved within the vertebral body, relative positions between said at least two radiopaque markers is viewable on radiography to determine an extent of the curve.

    The system of any one of claims 1-7, wherein said introducer device further comprises a housing, and a locking mechanism operably coupling said housing and said actuator, wherein said locking mechanism is configured to permit selective locking of said actuator in one of a plurality of positions.

    A system for augmenting a vertebral body, said system comprising:an access cannula comprising a cannula hub, and a cannula shaft extending from said cannula hub with said cannula shaft comprising a distal end positionable within the vertebral body and defining a lumen along a longitudinal axis;an introducer device comprising: an actuator configured to receive an input from a user;a shaft comprising a rigid proximal portion coupled to said actuator and defining a proximal end of said shaft, and a flexible distal portion, wherein a length of said shaft between said proximal end and a distal end is sufficient for said shaft to extend through and be operable beyond said distal end of said access cannula, wherein said flexible distal portion comprises a pre-set curve in an unconstrained state;a pulling element coupled to said actuator and to said shaft at or near said distal end with said pulling element extending along at least a portion of said pre-set curve, wherein altering tension on said pulling element in response to the input to provided to said actuator is configured to move said pre-set curve between said unconstrained state and a constrained state in which said flexible distal portion at least partially straightens;and a flexible sheath at least partially overlying said shaft with said flexible sheath comprising a distal end positionable near said distal end of said shaft such that said flexible sheath is configured to extend through and be operable beyond said distal end of said access cannula with a distal portion of said flexible sheath conforming to said flexible distal portion as said pre-set curve moves between said constrained state and said unconstrained state, wherein said introducer device is removable from said flexible sheath with said distal end of said flexible sheath remaining at the target site offset from said longitudinal axis.

    A system for augmenting a vertebral body, said system comprising:an access cannula comprising a cannula hub, and a cannula shaft extending from said cannula hub and comprising a distal end positionable within the vertebral body;a delivery cannula comprising a delivery hub, a sheath extending from said delivery hub and comprising a distal end opposite a proximal end collectively defining a length sufficient to extend through and be operable beyond said distal end of said access cannula, wherein said delivery hub is movable relative to said cannula hub such that said sheath is slidably disposed within said cannula shaft;an expandable member assembly comprising a balloon hub adapted to be coupled to a fluid source, a balloon tube extending from said balloon hub and sized to be slidably inserted within said sheath, and a balloon coupled to a distal end of said balloon tube and configured to be inflated with fluid from the fluid source to displace cancellous bone within the vertebral body;and a spacer hub configured to facilitate proximal movement of said delivery cannula relative to said access cannula and said expandable member assembly to expose said balloon within the vertebral body through a syringe-style input from a user, said spacer hub comprising a distal portion engaging said cannula hub and a proximal portion for engaging said balloon hub with said distal and proximal portions comprising opposing stop surfaces defining a void space with said delivery' hub configured to be movably disposed within said space such that said opposing stop surfaces provide a terminus of movement of said delivery hub.

    The system of any one of claims 11 and 12, wherein said delivery hub defines a lumen and each of said distal and proximal portions of said spacer hub define coaxial apertures with at least a portion of said balloon tube extending through said lumen and said coaxial apertures such that said delivery hub is slidable along said balloon tube between said distal and proximal portions of said spacer hub.

    The system of any one of claims 11-13, wherein said spacer hub further comprises at least one side extending between said distal and proximal portions.

    The system of any one of claims 11-15, wherein said spacer hub further comprises a pivot pivotably coupling said distal portion and said proximal portion.

    The system of any one of claims 11-17, wherein said cannula hub further comprises a shaft hub rigidly coupled to said cannula shaft to provide a datum, and a tuning hub movably coupled to said shaft hub and comprising an interference surface for engaging said distal portion of said spacer hub, wherein movement of said tuning hub relative to said shaft hub facilitates selective adjustment of the axial position of said interference surface relative to the datum.

    A system for augmenting a vertebral body, said system comprising:an access cannula comprising a cannula hub, and a cannula shaft extending from said cannula hub and comprising a distal end positionable within the vertebral body;a delivery cannula comprising a delivery hub, a sheath extending from said delivery hub and comprising a distal end opposite a proximal end collectively defining a length sufficient to extend through and be operable beyond said distal end of said access cannula, wherein said delivery hub is movable relative to said cannula hub such that said sheath is slidably disposed within said cannula shaft;an expandable member assembly comprising a balloon hub adapted to be coupled to a fluid source, a balloon tube extending from said balloon hub and sized to be slidably inserted within said sheath, and a balloon coupled to a distal end of said balloon tube and configured to be inflated with fluid from the fluid source to displace cancellous bone within the vertebral body;and a spacer hub configured to facilitate proximal movement of said delivery cannula relative to said access cannula and said expandable member assembly to expose said balloon within the vertebral body through a syringe-style input from a user, said spacer hub comprising a distal portion engaging said cannula hub, a proximal portion configured to engage said balloon hub, and a pivot pivotably coupling said distal portion to said proximal portion.

    A system for augmenting a vertebral body, said system comprising:an access cannula comprising: a cannula shaft comprising a distal end positionable within the vertebral body and defining a lumen along a longitudinal axis;a cannula hub comprising a shaft hub rigidly coupled to said cannula shaft to provide a datum, and a tuning hub movably coupled to said shaft hub to a plurality of supported positions with said tuning hub comprising an interference surface;an instrument comprising an elongate member slidably disposed within said lumen of said cannula shaft and comprising a length defined between proximal and distal ends being sufficient for said elongate member to extend through and be operable beyond said distal end of said access cannula, and an instrument hub coupled to said proximal end of said elongate member with said cannula hub configured to be engaged by said instrument hub to prevent distal movement of said instrument relative to said access cannula while permitting proximal movement of said instrument relative to said access cannula, wherein movement of said tuning hub relative to said shaft hub to one of said plurality of supported positions facilitates selective adjustment of an axial position of said interference surface of said tuning hub relative to the datum.

    The system of any one of claims 21 and 22, wherein each of said shaft hub and said tuning hub define apertures coaxially aligned with said lumen with at least one of said instrument hub and said elongate member extending through said coaxial apertures.

    The system of any one of claims 21-23, wherein said instrument is one of an introducer device, a delivery cannula, and an expandable member assembly.

    A system for augmenting a vertebral body, said system comprising:an access cannula comprising a distal end positionable within the vertebral body and defining a lumen along a longitudinal axis;an introducer device comprising: an actuator;a shaft comprising a rigid proximal portion coupled to said actuator and defining a proximal end of said shaft, and a flexible distal portion at or near a distal end opposite said proximal end with a length of said shaft between said proximal and distal ends being sufficient for said shaft to extend through and be operable beyond said distal end of said access cannula, wherein said flexible distal portion is configured to move between a plurality of curved configurations within the vertebral body to target sites offset from said longitudinal axis;a flexible sheath at least partially overlying said shaft with said flexible sheath comprising a distal end positionable near said distal end of said shaft such that said flexible sheath is configured to extend through and be operable beyond said distal end of said access cannula with at least a distal portion of said flexible sheath configured to conform to said flexible distal portion as said flexible distal portion moves between said curved configurations;and at least two radiopaque markers disposed on said distal portion of said flexible sheath and spaced apart from one another such that, as said distal portion is curved within the vertebral body, relative positions between said at least two radiopaque markers is viewable on radiography to determine extent of curvatures of said curved configuration.

    The system of any one of claims 25 and 26 wherein said at least two radiopaque markers are one of dots, bands, rings, and lines.

    The system of any one of claims 25-27, wherein said flexible distal portion of said introducer comprises a pre-set to define an inner curved surface opposite an outer curved surface, and a pulling element coupled to said shaft at or near said distal end with said pulling element extending along at least a portion of said outer surface of said pre-set curve, wherein said pulling element is configured to be tensioned to move said pre-set curve between said curvedconfigurations.

    The system of any one of claims 25-27, wherein said flexible distal portion of said introducer comprises a pulling element coupled to said shaft at or near said distal end with said pulling element, wherein altering tension on said pulling element moves said flexible distal portion between said curved configurations.

    The system of any one of claims 25-27, wherein said flexible distal portion of said introducer comprises shape memory metal biased towards a pre-set curve in said curved configuration, wherein said access cannula is configured to constrain said pre-set curve to a straightened configuration as said shaft is directed through said access cannula.
