    A device (314) comprising:a sensing circuit (386) configured to receive a cardiac electrical signal;a therapy delivery circuit (384) configured to deliver an electrical stimulation therapy;a telemetry circuit (388) configured for communication with a ventricular assist device (VAD);and a control circuit (380) coupled to the sensing circuit, the therapy delivery circuit and the telemetry circuit and configured to: detect a cardiac event from the cardiac electrical signal received by the sensing circuit;receive a confirmation signal transmitted by the VAD (100);select a therapy delivery response based on the received confirmation signal;and control the therapy delivery circuit according to the selected therapy delivery response.

    The device of any one of claims 2-3, wherein the control circuit is configured to control the therapy delivery circuit to start charging a high voltage capacitor after transmitting the request signal and prior to receiving the confirmation signal.

    The device of any one of claims 1-4, wherein the control circuit is configured to detect the cardiac event by detecting a shockable rhythm.

    The device of any one of claims 1-5, wherein the control circuit is configured to select the therapy delivery response based on the received confirmation signal by one of:scheduling a shock therapy in response to the confirmation signal indicating a shockable flow condition detected by the VAD, or withholding a shock therapy in response to the confirmation signal indicating a non-shockable flow condition detected by the VAD.

    The device of any one of claims 1-6, wherein the control circuit is configured to select the therapy delivery response by withholding an electrical stimulation therapy based on the received confirmation signal not confirming the detected cardiac event.

    The device of any one of claims 1-7, wherein the control circuit is configured to select the therapy delivery response by controlling the therapy delivery circuit to deliver an electrical stimulation therapy based on the confirmation signal confirming the detected cardiac event.

    A system comprising:a blood pump (101) comprising a motor, an inflow cannula (104) for receiving blood from a patient's heart, and a flow outlet (106) for coupling to a patient's artery;a controller (110) comprising a power source (162), a processor (150) and a telemetry circuit (158) configured for communication with a second device (314);and a drive line (120) coupled between the blood pump and the controller for transmitting a drive signal to the blood pump;wherein the controller is configured to: estimate a flow rate of the blood pump;detect a cardiac event from the estimated flow rate;and control the telemetry circuit to transmit a request signal to the second device in response to detecting the cardiac event.

    The system of one of claims 9-10, wherein the controller is configured to:detect the cardiac event by comparing the estimated flow rate to a low flow threshold;in response to the estimated flow rate being less than the low flow threshold, compare the estimated flow rate to shockable rhythm criteria;and transmit the request signal to the second device in response to the shockable rhythm criteria being met.

    The system of one of claims 9-11, wherein the telemetry circuit is configured to receive a response signal from the second device, the response signal comprising one of:a normal rhythm signal in response to an arrhythmia not being detected based on the cardiac signal analysis;or an arrhythmia detection signal in response to an arrhythmia being detected based on the cardiac signal analysis;and wherein the controller is configured to select a response to the detected cardiac event based on the response signal received from the second device.

    The system of one of claims 9-13, wherein the controller is configured to one of:generate a patient alarm in response to detecting the cardiac event, and/or adjust a speed of the blood pump in response to detecting the cardiac event.
