    Claims of equivalent WO 2012103196 A2 What is claimed is:1. A device for stimulating a hypoglossal nerve, comprising: a neurostimulator for generating an electrical signal and measuring a bio- impedance signal;a stimulation lead connected to the neurostimulator, the stimulation lead having a nerve cuff electrode configured for delivering the electrical signal to the hypoglossal nerve;a respiration sensing lead having a proximal portion with a proximal end connected to the neurostimulator and a distal end terminating in a bifurcation with first and second distal portions extending in different directions therefrom, wherein the first and second distal portions are configured to extend over opposite sides of the thorax.

    A method of delivering therapy to a subject, comprising:chronically implanting a neurostimulator connected to a stimulation delivery lead;titrating a stimulation level of the neurostimulator while the subject is asleep on a first night to determine a range of effective stimulation levels, wherein the effective range has a lower bound and an upper bound, wherein the upper bound corresponds to a stimulation level that causes arousal from sleep;delivering therapy at a first level while the subject is asleep on a second night subsequent to the first night, wherein the first level is below or within the effective range;and delivering therapy at a second level while the subject is asleep on a third night subsequent to the second night, wherein the second level is greater than the second level and within the effective range.

    A method of delivering therapy to a subject, comprising:chronically implanting a neurostimulator connected to a stimulation delivery lead, the stimulation delivery lead connected to a hypoglossal nerve;programming the neurostimulator using a programmer to enable a limited range of stimulation settings to be adjusted by the subject;providing a therapy controller to the subject;and adjusting the stimulation level using the therapy controller to a level within the pre-programmed range.

    A method of delivering therapy to a subject, comprising:chronically implanting a neurostimulator connected to a stimulation delivery lead, the stimulation delivery lead connected to a hypoglossal nerve;programming the neurostimulator using a programmer to enable a first therapeutic level and a second therapeutic level, wherein the second therapeutic level is greater than the first therapeutic level;delivering therapy to the subject at the first level during a first portion of a sleeping period;and delivering;therapy to the subject at the second level during a second portion of the sleeping period, wherein second portion is subsequent to the first portion.

    A device for stimulating a hypoglossal nerve, comprising:a neurostimulator for generating an electrical signal;a stimulation lead connected to the neurostimulator, the stimulation lead having a nerve cuff electrode configured for delivering the electrical signal to the hypoglossal nerve;and wherein the electrical signal comprises a pulse train envelope of two or more pulses at two or more stimulation levels.

    A device for stimulating a hypoglossal nerve, comprising:a neurostimulator for generating an electrical signal;a stimulation lead connected to the neurostimulator, the stimulation lead having a nerve cuff electrode configured for delivering the electrical signal to the hypoglossal nerve and configured for monitoring neural activity of the hypoglossal nerve;and wherein the device may change stimulation level based on the monitored neural activity.

    A device for stimulating a hypoglossal nerve, comprising:an oxygen sensor;a neurostimulator for generating an electrical signal;a stimulation lead connected to the neurostimulator, the stimulation lead having a nerve cuff electrode configured for delivering the electrical signal to the hypoglossal nerve;and wherein the device may change stimulation level based on oxygen sensor reading.

    A device for stimulating a hypoglossal nerve, comprising:an accelerometer;a neurostimulator for generating an electrical signal;a stimulation lead connected to the neurostimulator, the stimulation lead having a nerve cuff electrode configured for delivering the electrical signal to the hypoglossal nerve;and wherein the device may change stimulation level based on the accelerometer reading.

    A method of delivering therapy to a subject, comprising:chronically implanting a neurostimulator connected to a stimulation delivery lead, the stimulation delivery lead connected to a hypoglossal nerve;programming the neurostimulator using a programmer to enable a first stimulation level and a second stimulation level, wherein the second therapeutic level is different than the first therapeutic level;delivering therapy to the subject at the first level during a sleeping period;measuring a parameter during the first sleeping period;delivering therapy to the subject at the second level during a second sleeping period, wherein the second sleeping period is subsequent to the first;measuring the parameter during the second sleeping period;and comparing the measures to adjust the stimulation level.

    A method of delivering therapy to a subject, comprising:chronically implanting a neurostimulator connected to a stimulation delivery lead, the stimulation delivery lead connected to a hypoglossal nerve;providing a therapy controller to the subject;programming the neurostimulator using a programmer to enable the subject to pause stimulation using the therapy controller;and programming the neurostimulator using a programmer to adjust the duration of the pause as a function of the number of pauses.

    A method of delivering therapy to a subject having a hypoglossal nerve with a first nerve fiber having a first capture threshold and second nerve fiber having a second capture threshold, comprising:chronically implanting a neurostimulator connected to a stimulation delivery lead with a first and second electrode on the stimulation delivery lead, the stimulation delivery lead connected to the hypoglossal nerve;delivering stimulation to the first nerve fiber from the first electrode at a level less than the first nerve fiber capture threshold;and delivering stimulation to the second nerve fiber from the second electrode at a level greater than the second nerve fiber capture threshold.
