    A system for establishing an intramedullary path, comprising:a body (204) defining a first aperture (206) that extends through the body (204), the first aperture (206) sized and configured to receive a surgical tool therethrough;and a first bone engaging structure (222) extending from the body (204) in a first direction, the first bone engaging structure (222) including a first surface (228);wherein when the first surface (228) of the bone engaging structure (222) engages a surface topography of a first bone, characterized in that the body (204) is sized and configured to be received within a resected bone space (22) and is custom manufactured, whereby the first surface (228) of the first bone engaging structure (222) is complementary to a surface topography of a first bone and an axis defined by the first aperture (206) is substantially collinear with a mechanical axis of the first bone.

    System of claims 1 or 2, wherein the first bone engaging structure (222) defines one or more holes (230) for receiving a k-wire or pin.

    System of claims 1 to 3, wherein a second bone engaging structure (224) extends from the body (204) in a direction that is substantially opposite the first direction, the second bone engaging structure (224) including a second surface (238) that is complementary to a surface topography of a second bone.

    System of claims 4 or 5, wherein the second bone engaging structure (224) defines one or more holes (236) for receiving a k-wire.

    System of claims 1 to 6, further comprising:a drill guide (202) sized and configured to be received within the first aperture (206) defined by the body (204), the drill guide (202) defining a second aperture (246) sized and configured to receive the surgical tool therethrough.

    System of claims 7 or 8, wherein an outer surface of the drill guide (202) includes a flat surface (242) that is complementary to a non-rotation feature (220) disposed within the first aperture (206).

    System of claims 7 to 9, wherein the second aperture (246) tapers along at least part of its length.

    System of claims 1 to 10, comprising - a foot holder assembly (302) having a pair of alignment rods (318);- a coupling bar (400) sized and configured to engage the pair of alignment rods (318) of the foot holder assembly (300), wherein the coupling bar (400) is configured to be coupled to the body (204) of the drill guide mount (200) for maintaining a position of the drill guide mount (200) relative to the foot holder assembly (302).

    System of claims 11 or 12 when dependent from claim 7, wherein the foot holder assembly (302) comprises a back plate (312) which carries a bushing (324) or an aperture (503), the aperture (246) of the drill guide (202) disposed within the body (202) being aligned with an axis defined by the bushing (324) or by the aperture (503).
