    A medication delivery device (1) comprising:- a housing (3) having a proximal and a distal end, - a medication receptacle (2) designed to be engaged with the distal end of the housing (3) by engagement of first engagement means (5) with second engagement means (4) and - a dosing mechanism including • a piston rod (17) which is moveable in a distal direction for medication delivery and • a drive device for moving the piston rod (17) in the distal direction for medication delivery;- wherein the medication receptacle (2) comprises an actuation means (70) for bringing a guiding means (11) in a guiding position in which the guiding means (11) guides the piston rod (17) for medication delivery, wherein the first engagement means (5) and the second engagement means (4) are moveable into engagement by a movement of the medication receptacle (2) which ends in a rotational movement without axial movement of the medication receptacle (2) with respect to the housing (3), the actuation means (70) thereby bringing the guiding means (11) in the guiding position, wherein the guiding means is brought into the guiding position during the rotational movement of the medication receptacle while the medication receptacle does not move axially with respect to the housing, characterized in that when the guiding means (11) is in the guiding position the piston rod (17) can be moved axially in the distal direction and rotated in one direction with respect to the housing (3) by the drive device and the piston rod (17) is prevented from axial movement in the proximal direction with respect to the housing (3) when the guiding means (11) is not in the guiding position, the piston rod (17) is free move axially in the proximal direction with respect to the housing (3) while rotating in the other direction.

    The medication delivery device (1) according to one of claims 1 to 3, characterized in that the guiding means (11) is a nut means threadedly engaged with the piston rod (17).

    The medication delivery device (1) according to any of claims 1 to 4, characterized in that - the drive device comprises a drive sleeve (19) and - the piston rod (17) comprises two threaded sections (15, 16), wherein a first threaded section (15) is provided for threaded engagement with the guiding means (11) and a second threaded section (16) is provided for threaded engagement with the drive sleeve (19) and wherein the pitch direction of the second threaded section (16) is oppositely directed to the pitch direction of the first threaded section (15).

    The medication delivery device (1) according to claims 5 or 6, characterized in that the dosing mechanism comprises:- a dose dial sleeve (27) having a helical thread engaged with a helical thread of the housing (3), the drive sleeve (19) being releasably coupled with the dose dial sleeve (27) and - a clutch means (24) located between the dose dial sleeve (27) and the drive sleeve (19), wherein a) when the dose dial sleeve (27) and the drive sleeve (19) are coupled, both are allowed to rotate with respect to the housing (3), and b) when the dose dial sleeve (27) and the drive sleeve (19) are de-coupled, rotation of the dose dial sleeve (27) with respect to the housing (3) is allowed, whilst rotation of the drive sleeve (19) with respect to the housing (3) is not allowed and axial movement of the drive sleeve (19) is allowed in the distal direction thereby transferring a force in the distal direction to the piston rod (17).

    The medication delivery device (1) according to any of claims 1 to 7, characterized in that the medication delivery device (1) comprises a locking means (9) which is non-rotatable with respect to the housing (3) and which is engageable with the guiding means (11), thereby locking the guiding means (11) for preventing rotation of the guiding means (11) with respect to the housing (3).

    The medication delivery device (1) according to claims 8 or 9, characterized in that the guiding means (11) or the locking means (9) comprises a shape which interacts with the shape of the actuation means (70) for driving the locking means (9) and the guiding means (11) into engagement by the rotational movement of the actuation means (70).

    The medication delivery device (1) according to any of claims 8 to 10, characterized in that at least one inclined surface is arranged on the actuation means (70) and at least one correspondingly inclined surface (39) is arranged on the guiding means (11) or on the locking means (9), the inclined surfaces being formed such that for interaction of the actuation means (70) and the guiding means (11) or of the actuation means (70) and the locking means (9) the inclined surfaces glide along each other.

    The medication delivery device (1) according to one of claims 1 to 12, characterized in that the guiding means (11) is a nut means which is threadedly engaged with the piston rod (17) and which is engaged with the locking means in the guiding position.

    The medication delivery device (1) according to any of claims 8 to 13, characterized in that locking means (9) and the guiding means (11) comprise face teeth (10, 12) which interlock when the locking means (9) and the guiding means (11) are engaged in the guiding position.

    The medication delivery device (1) according to any of claims 8 to 14, characterized in that the locking means (9) and the guiding means (11) disengage under the force of a biasing means (14) when the medication receptacle (2) is disengaged from the housing (3).

    The medication delivery device (1) according to any of claims 8 to 15, characterized in that when the medication receptacle (2) is disengaged from the housing (3), the locking means (9) and the guiding means (11) are disengaged, whereby the guiding means (19) is free to rotate with respect to the housing (3).

    The medication delivery device (1) according to one of claims 1 to 16, characterized in that the medication receptacle (2) is a cartridge holder which is designed to receive a cartridge (6) filled with medication.

    The medication delivery device (1) according to one of claims 1 to 17, characterized in that the medication receptacle (2) or an insert of the medication receptacle (2) comprises the first engagement means (5) and that the housing (3) or an insert (3A) of the housing (3) comprises the second engagement means (4).

    The medication delivery device (1) according to one of claims 1 to 18, characterized in that the first engagement means (5) is a thread of the medication receptacle (2) and the second engagement means (4) is an engaging element of the housing (3) or of an insert (3A) of the housing (3) for engagement with the thread of the medication receptacle, wherein the distal end of the thread of the medication receptacle (2) merges into a annular groove such that the medication receptacle (2) is engaged with the housing (3) by first rotating and moving proximally with respect to the housing (3) and then only rotating with respect to the housing (3).

    The medication delivery device (1) according to one of claims 1 to 18, characterized in that the distal end of the housing (3) is provided with an insert (3B) comprising the second engagement means (4) designed to engage with the first engagement means (5) at the proximal end of the medication receptacle (2), the insert (3B) being secured against rotation but being free to move axially with respect to the housing (3), wherein distal axial movement of the insert (3B) is limited by a retaining means (63).

    The medication delivery device according to one of claims 1 to 21, characterized in that the medication receptacle (2) and the housing (3) are provided with snap-in features by means of which the medication receptacle (2) and the housing (3) are held in engagement during medication delivery.

    A medication delivery device (1) comprising:- a housing (3) having a proximal and a distal end. - a medication receptacle (2) designed to be engaged with the distal end of the housing (3) by engagement of first engagement means (5) with second engagement means (4) and - a dosing mechanism including • a piston rod (17) which is moveable in a distal direction for medication delivery and • a drive device for moving the piston rod (17) in the distal direction for medication delivery: wherein the medication receptacle (2) comprises an actuation means (70) for bringing a guiding means (11) in a guiding position in which the guiding means (11) guides the piston rod (17) for medication delivery, wherein the first engagement means (5) and the second engagement means (4) are moveable into engagement by a movement of the medication receptacle (2) which ends in a rotational movement without axial movement of the medication receptacle (2) with respect to the housing (3), the actuation means (70) thereby bringing the guiding means (11) in the guiding position. characterized in that the guiding means (11) is a nut means threadedly engaged with the piston rod (17), wherein the medication delivery device (1) comprises a locking means (9) which is non-rotatable with respect to the housing (3) and which is engageable with the guiding means (11), thereby locking the guiding means (11) for preventing rotation of the guiding means (11) with respect to the housing (3), and wherein at least one inclined surface is arranged on the actuation means (70) and at least one correspondingly inclined surface (39) is arranged on the guiding means (11) or on the locking means (9), the inclined surfaces being formed such that for interaction of the actuation means (70) and the guiding means (11) or of the actuation means (70) and the locking means (9) the inclined surfaces glide along each other.

    The medication delivery device (1) according to any of claims 1 to 23 which is a pen-type device.

    The medication delivery device (1) according to any of claims 1 to 24 which is an injector-type device.
