    A medical system comprising an over-tube (1) having at least one channel (15a, 15b, 15c) extending from a proximal portion (5) to a distal portion (4) and having a distal opening (41) through which a tool (2, 3) is able to be inserted and guided into a body cavity, characterized in that the over-tube (1) comprises a restraining mechanism (45, 54;61, 62) for restraining a relative movement between the tool (2, 3) inserted through the over-tube (1) and the over-tube (1), whereby a desired position of the tool (2, 3) projecting from the distal opening (41) of the over-tube (1) is fixable relative to the over-tube (1) by an operator.

    A medical system according to Claim 1, wherein the restraining mechanism comprises:an engagement mechanism (45;61) provided at a distal portion of the over-tube for releasably engaging the tool inserted through the over-tube with the over-tube;and an operating mechanism (54-58;62-65) for operating engagement and release of the engagement mechanism.

    A medical system according to Claim 2, wherein the operating mechanism (54-58;62-65) is provided on a proximal side of the over-tube.

    A medical system according to Claim 2, wherein the engagement mechanism (45, 54) comprises:a resilient strip (46) disposed at the distal portion of the over-tube;and a pressing member (48) for pressing the resilient strip (46) against the tool (2, 3) inserted through the over-tube (1).

    A medical system according to Claim 4, wherein the resilient strip (46) engages the tool (2, 3) by being deformed by depression by the pressing member (48), and releases the engagement by being restored by releasing of the depression.

    A medical system according to Claim 5, wherein the pressing member (48) presses the resilient strip (46) and releases depression from the resilient strip by sliding movement along the inner wall of the channel (15a, 15b, 15c) of the over-tube (1), and wherein the operating mechanism (54-58) operates the sliding movement of the pressing member (48).

    A medical system according to Claim 2, wherein the engagement mechanism includes a balloon (61) disposed at the distal end of the over-tube.

    A medical system according to Claim 7, wherein the operating mechanism (62-65) feeds and discharges fluid to and from the balloon (61).

    A medical system according to Claim 2, wherein the operation of the operating mechanism (54-58) is transmitted to the engagement mechanism via a wire (54).

    A medical system according to Claim 2, wherein the operation of the operating mechanism (62-65) is transmitted to the engagement mechanism via a tube (62) through which a fluid is fedable and dischargeable.

    A medical system according to Claim 2, wherein the over-tube (1) includes a plurality of channels (15a, 15b, 15c) through which a plurality of tools (2, 3) can be inserted, and wherein the engagement mechanism (45;61) is provided in at least one of the channels (15a, 15b, 15c).

    A medical system according to Claim 11, wherein the engagement mechanism (45;61) is provided in at least two of the plurality of channels (15a, 15b, 15c) and each of the at least two of the plurality of channels (15a, 15b, 15c) is provided with a respective engagement mechanism (45;61) which can be operated independently of each other.

    A medical system according to Claim 1, further comprising a supporting device (70;102) for supporting the over-tube (1) outside the body cavity, the supporting device (70;102) being adapted to fix the proximal portion of the over-tube (1) in position.

    A medical system according to Claim 13, wherein the supporting device (70;102) comprises a first holder (71a, 71b;104) for supporting the tool (2, 3) at a first portion thereof and a second holder (72a, 72b;105) for supporting the tool (2, 3) at a second portion thereof.

    A medical system according to Claim 14, wherein the supporting device (70) further comprises a first supporting position adjusting mechanism (76, 77) being capable of changing the supporting position of the over-tube (1) by the first holder (71a, 71b).

    A medical system according to Claim 14, wherein the supporting device (70) further comprises a second supporting position adjusting mechanism (81, 82) being capable of changing the supporting position of the over-tube (1) by the second holder (72a, 72b).

    A medical system according to Claim 14, wherein the supporting device (70;102) can change the relative position of the first holder (71a, 71b;104) and the second holder (72a, 72b;105).

    A medical system according to Claim 18, further comprising an applying mechanism (113;124) for applying a fixing force for determining and fixing the position of the second holder (105;122) with respect to the first holder (104;121) and a releasing mechanism for releasing the fixing force.

    A medical system according to Claim 19, further comprising an operating mechanism for selectively operating the applying mechanism and the releasing mechanism.

    A medical system according to Claim 1, wherein the medical system comprises an endoscope (3) as the tool, and wherein the restraining mechanism (45, 54;61, 62) is a connecting member for connecting the distal portion of the over-tube (1) and the distal portion of the endoscope (3).

    A medical system according to Claim 1, wherein the tool (2, 3) to be inserted through the over-tube (1) includes an endoscope (3).

    A medical system according to Claim 1, wherein the tool (2, 3) to be inserted through the over-tube includes an instrument insertion guide tube (2).
