EP1570789A2

Surgical manipulator for a telerobotic system

Abstract

A surgical system and surgical instruments for a telerobitic system are disclosed. In one embodiment, the surgical instrument has an elongate shaft having a proximal portion, a distal portion, an inner lumen, and at least one axially extending slot. An end effector is disposed adjacent the distal portion of the elongate shaft and an actuator assembly, comprising a linkage, is disposed within the inner lumen. A movable actuator pin extends laterally from the linkage and through the axially extending slot to releasably couple the actuator assembly to a driver of a robotic surgical instrument holder. The system disclosed may enable the rapid engagement and disengagement of surgical instruments from an instrument holder during surgical procedures.

21 claims: 11 independent, 10 dependent

  1. 1
A surgical instrument comprising:an elongate shaft having a proximal portion, a distal portion, an inner lumen, and at least one axially extending slot;andan end effector disposed adjacent the distal portion of the elongate shaft;and an actuator assembly comprising a linkage disposed within the inner lumen and a movable actuator pin laterally extending from the linkage and extending through the axially extending slot to releasably couple the actuator assembly to a driver of a robotic surgical instrument holder.
  2. 4
The surgical instrument of any preceding claim wherein said end effector includes jaws that are movable between an open position where the jaws are disposed apart and a closed position where the jaws are disposed closer together.
  3. 7
The surgical instrument of any of claims 4 to 6 wherein said instrument further comprises means for biasing said jaws towards the closed position.
  4. 8
The surgical instrument of any preceding claim wherein the elongate shaft includes a pair of opposed axially extending slots and the actuator pin extends laterally through the pair of slots.
  5. 9
The surgical instrument of any preceding claim wherein the actuator pins are releasably received within an aperture in the driver.
  6. 10
The surgical instrument of any preceding claim wherein the driver is coupled to the instrument holder.
  7. 11
The surgical instrument of any preceding claim wherein the actuator pin is releasably received by an aperture of the instrument holder.
  8. 12
The surgical instrument of any preceding claim wherein the end effector is coupled to the elongate shaft through a plurality of joints.
  9. 13
A surgical system comprising:a surgical instrument comprising an elongate shaft with proximal and distal ends and a mounting means laterally extending from the shaft intermediate the proximal and distal ends;an instrument holder for releasably holding the surgical instrument, the instrument holder comprising a body and an instrument support movably mounted on said body and having an interface engageable with the surgical instrument to releasably mount the instrument to said instrument holder;anda device for moving said instrument about a desired spherical center of rotation at a desired location along said instrument, the device comprising: a base for supporting said instrument holder;a first linkage, including at least a first rod, pivotally mounted on said base for rotation about an axis, said instrument holder holding said instrument in a position so that the desired remote center of spherical rotation along the instrument intersects said axis;anda second linkage, including at least a second rod, connected to said instrument holder for moving said instrument holder in a parallel-plane relationship to said second linkage, said second linkage being pivotally connected to said first linkage so that said second rod remains parallel to said axis and said first rod remains parallel to the instrument, such that said spherical center of rotation is maintained at the desired fixed location.
  10. 17
The system of any of claims 13 to 16 wherein said device for moving said instrument about a desired spherical center of rotation is fixable to a surface at an orientation with respect to the vertical, the system further including an inclinometer mounted to said device for measuring said orientation.
  11. 18
A surgical instrument comprising:an elongated shaft having a proximal end, a distal end and a mechanical coupling therebetween;an end effector coupled to the distal end for engaging tissue;mounting means affixed to and having a protrusion laterally extending from said shaft between the proximal and distal ends for releasably holding the instrument;anda movable actuator mounted to the shaft and operatively coupled to the end effector for manipulating the end effector, the actuator including coupling means for cooperating with an external driver to controllably move the actuator, the coupling means being spaced from and axially movable with respect to the mounting means.
