Apparatus for atrioventricular valve repair

Abstract

Methods and devices are disclosed for minimally invasive procedures in the heart. In one application, a catheter is advanced from the left atrium through the mitral valve and along the left ventricular outflow tract to orient and stabilize the catheter and enable a procedure such as a “bow tie” repair of the mitral valve. Right heart procedures are also disclosed.

    A catheter (11) for accessing a beating heart, comprising:an elongate flexible body, having a proximal end (13) and a distal end (11);an anchor zone on a distal portion of the flexible body;and at least one tissue grasper (22,23) carried by the flexible body proximally of the anchor zone, the tissue grasper (22, 23) configured for grasping a heart valve leaflet characterised in that the shape of the catheter (11) is adapted to act as a fulcrum about which manipulation of the catheter (11) with respect to individual valve leaflets is facilitated.

    The catheter (11) according to Claim 1, wherein the minimum length of the anchor zone on the distal side of the tissue grasper (22, 23) is at least about 3 cm.

    The catheter (11) according to Claim 1, wherein the minimum length of the anchor zone on the distal side of the tissue grasper (22,23) is at least about 5 cm.

    The catheter (11) according to Claim 1, wherein the minimum length of the anchor zone on the distal side of the tissue grasper (22,23) is at least about 10 cm.

    The catheter (11) according to any of Claims 1 to 4, wherein the tissue grasper (22,23) is moveable between an axial orientation for transluminal navigation and an inclined orientation for grasping a heart valve leaflet (3,4).

    The catheter (11) according to any of Claims 1 to 5, comprising a first and a second tissue grasper (22,23).

    The catheter according to Claim 6, wherein the second tissue grasper comprises a tissue grasper (24,25) for grasping a heart valve leaflet.

    The catheter (11) according to Claim 1, comprising at least a first component which is axially moveable with respect to a second component.

    The catheter (11) according to any of claims 1 to 8, designed to access the beating heart via the left atrium, across the mitral valve, through the left ventricular outflow tract and the aortic valve, whereby the catheter (11) is automatically alignable between the chordal attachments to the papillary muscles.

    The catheter (11) according to any of the preceding claims having an inflatable balloon (16) at the distal end, a central lumen for a guide wire (17), which catheter (11) is advanceable into the ascending aorta from the left atrium.

    The catheter (11) according to any of the preceding claims wherein the tissue grasper (22, 23) is configured to be inclined radially outward in a direction.

    The catheter (11) according to any of the preceding claims containing pre-shaped curve to assist in advancement through the left ventricle into the aorta.

    The catheter (11) according to any of claims further comprising a receptacle (28) located within the first tissue grasper (22) for receiving a first fixating member (24), preferably further comprising a second receptacle (29) located within the second tissue grasper (23) for receiving a second fixating member (25), more preferably wherein a first end of a fastening material (36) is coupled with the first receptacle (28) and second end of the fastening material (30) is coupled with the second receptacle (24).

    The catheter (11) according to any of the preceding claims wherein the anchor zone is of sufficient length distally of the procedure zone, to enable orientation and anchoring of its catheter with the vasculature.
