    A system adapted for use in at least one of a first ear (101A) having a first ear canal (303) and a second ear (101B) having a second ear canal (303) comprising:a first ear-level device (110, 210A) configured to be one of an in-the-ear (ITE) device and a behind-the-ear (BTE) device, the first ear-level device including: a first microphone (440) adapted for use in one of the first ear canal and the second ear canal, and adapted to receive an occluded sound (305) within the one of the first ear canal (303) and the second ear canal (303);and a wireless receiver, connected to a first processor (460), to receive a first wireless signal representing a remote sound;wherein the first processor (460) is connected to the first microphone (440) and is adapted to process the occluded sound (305) and the remote sound;the first ear level device further including: a speaker adapted for use in the one of the first ear canal (303) and the second ear canal (303) and adapted to transmit the processed remote sound to the one of the first ear canal (303) and the second ear canal (303);a wireless transmitter, connected to the first processor (460) and adapted to receive the processed occluded sound from the first processor (460) and transmit a second wireless signal representing the occluded sound;a first earmold housing at least the first microphone (440) and the speaker (470), at least a portion of the first earmold configured for placement in the one of the first ear canal (303) and the second ear canal (303);and a first voice operated exchange (VOX) circuit (430) connected to the first processor (460);the first VOX circuit (430) including: an occluded sound gating module (441) adapted to gate the occluded sound on, and to gate the remote sound off, when the occluded sound is detected while the remote sound is not detected;and a remote sound gating module (451) adapted to gate the remote sound on, and to gate the occluded sound off, when the incoming remote sound is detected while the occluded sound is not detected.

    A system according to any of the preceding claims, wherein the first processor (460) comprises a speech recognition module.

    A system according to any of claims 4 - 7, wherein the first earmold houses the first microphone (440), the first processor (460), the wireless transmitter, the wireless receiver, and the speaker (470), and wherein the first earmold is configured for use as the ITE device.

    A system according to any of claims 4 - 7, wherein the first ear level device (110,210A) comprises an ITE module including the first microphone (440) and the speaker (470) and a BTE module attached to the ITE module and including the second microphone (420), the first processor (460), the wireless transmitter and the wireless receiver.

    A system according to any of claims 7 - 13, further comprising a remote device (190, 290) communicatively coupled to the wireless transmitter and the wireless receiver via a telemetry link (195, 295) providing for simultaneous bidirectional communication between the first ear-level device (110, 210A) and the remote device (190, 290).

    A system according to any of claims 16 and 17, wherein the first ear-level device (110,210A) comprises the first voice operated exchange (VOX) circuit (430) connected to the first processor (460), and the second ear-level device (210B) further comprises a second VOX circuit (430) connected to the second processor.

    A system according to any of claims 16 - 18, wherein the first processor and the second processor each comprise a speech recognition module.

    A system according to any of claims 16 - 19, wherein the first and second ear-level devices are each the BTE device.

    A system according to any of claims 16 - 19, wherein the first and second ear-level devices are each the ITE device.

    A system according to any of claims 16 - 23, further comprising a remote device (190,290) communicatively coupled to the first and second ear-level devices.

    A method, comprising:detecting an occluded sound using a first microphone adapted for use in an ear canal (303);detecting an incoming radio signal representing a remote sound from a remote device (190, 290);starting a voice operated exchange (VOX) housed within an ear-level communication device when at least one of the occluded sound and the incoming radio signal is detected if the VOX is not already on, the ear level communication device being one of an in-the-ear (ITE) device and a behind-the-ear (BTE) device;gating the occluded sound on, and gating the remote sound off, when the occluded sound is detected while the incoming radio signal is not detected;and gating the remote sound on, and gating the occluded sound off, when the incoming radio signal is detected while the occluded sound is not detected.

    A method according to any of claims 26 and 27, wherein detecting the incoming radio signal comprises comparing an amplitude of the remote sound to a remote sound threshold, wherein the incoming radio signal is detected when the amplitude of the remote sound exceeds the remote sound threshold.

    A method according to any of claims 26 - 28, further comprising detecting an ambient sound using a second microphone, and wherein starting the VOX comprises starting the VOX when at least one of the occluded sound, the incoming radio signal, and the ambient sound is detected if the VOX is not already on.
