    A tissue retractor for percutaneous surgery in a patient, comprising:a first portion (22) having a proximal end (26) and a distal end (24);anda second portion (42) having a proximal end (46) and a distal end (44), said second portion forming with said first portion a working channel (50) in communication with an exterior of said first and second portions at said proximal ends and said distal ends with said working channel being enclosed by said first portion and said second portion between said distal and proximal ends;characterised in that the retractor further comprises a separation instrument (140) configured to enlarge said working channel by laterally moving' each of said first and second portions away from one another so as to separate the proximal ends and distal ends of the first and second portions, laterally, and to enlarge said working channel by pivoting each of said distal ends of said first and second portions about their proximal ends and away from one another such that only a portion of said working channel is enclosed by said first and second portions such that the working channel has a tapered configuration that reduces in size from the distal end of the retractor to the proximal end of the retractor;the retractor further comprising a guide member (200) extending between said first portion and said second portion, each of said first portion and said second portion being engageable with said guide member to secure said first portion and said second portion at any one of a number of pivotal orientations relative to said guide member;wherein the separation instrument (140) is engageable to said proximal ends of said first and second portions and is operable to move at least one of said first and second portions along said guide member;wherein said separation instrument has a lateral separator operable to laterally separate said first and second portions along said guide member and a rotational separator operable to pivot said first and second portions relative to said guide member. Ein Geweberetraktor für perkutane Chirurgie bei einem Patienten, der folgendes umfasst: einen ersten Abschnitt (22), der ein proximales Ende (26) und ein distales Ende (24) aufweist;undeinen zweiten Abschnitt (42), der ein proximales Ende (46) und ein distales Ende (44) aufweist, wobei der zweite Abschnitt mit dem ersten Abschnitt einen Arbeitskanal (50) bildet, der mit einem Äußeren des ersten und zweiten Abschnitts an den proximalen Enden und den distalen Enden in Verbindung steht, wobei der Arbeitskanal von dem ersten Abschnitt und dem zweiten Abschnitt zwischen den distalen und proximalen Enden umschlossen wird;dadurch gekennzeichnet, dass der Retraktor ferner ein Trenninstrument (140) umfasst, das zum Vergrößern des Arbeitskanals konfiguriert ist, indem jeweils der erste und zweite Abschnitt seitlich voneinander wegbewegt wird, um die proximalen Enden und distalen Enden des ersten und zweiten Abschnitts seitlich zu trennen und den Arbeitskanal durch Schwenken jeweils der distalen Enden des ersten und zweiten Abschnitts um ihre proximalen Enden herum und voneinander weg zu vergrößern, so dass nur ein Abschnitt des Arbeitskanals von dem ersten und zweiten Abschnitt umschlossen wird, so dass der Arbeitskanal eine verjüngte Konfiguration aufweist, deren Größe sich von dem distalen Ende des Retraktors zu dem proximalen Ende des Retraktors verringert,wobei der Retraktor ferner ein Führungselement (200) umfasst, das sich zwischen dem ersten Abschnitt und dem zweiten Abschnitt erstreckt, wobei jeweils der erste Abschnitt und der zweite Abschnitt mit dem Führungselement in Eingriff bringbar ist, um den ersten Abschnitt und den zweiten Abschnitt in einer beliebigen aus einer Anzahl von Schwenkausrichtungen relativ zu dem Führungselement zu sichern,wobei das Trenninstrument (140) mit den proximalen Enden des ersten und zweiten Abschnitts in Eingriff bringbar ist und betrieben werden kann, um mindestens einen von dem ersten und zweiten Abschnitt entlang des Führungselements zu bewegen,wobei das Trenninstrument ein seitliches Trennelement, das betrieben werden kann, um den ersten und zweiten Abschnitt entlang des Führungselements seitlich zu trennen, und ein drehbewegliches Trennelement aufweist, das betrieben werden kann, um den ersten und zweiten Abschnitt relativ zu dem Führungselement zu schwenken.
