    A device (20) for opening an occluded portion in a blood vessel of the vascular system of a living being, the device comprising a working head (32) at the distal end of an atherectomy catheter (22) and debris extraction means (30) comprising infusate pump means (84) for supplying infusate liquid and debris pump means (90) for removing the debris, wherein the debris pump means (90) is arranged to pump at a higher rate than the infusate pump means (84) characterised in that the operation of the two pump means (84, 90) is co-ordinated so that a change in the pump rate of one of the pump means (84 or 90) results in a change in the pump rate of the other of the pump means (90 or 84 respectively) and in that the device further comprises a guide catheter (24) through which said Atherectomy catheter (22) extends so that at least a portion of the infusate liquid passes through the annular clearance between the exterior of the atherectomy catheter (22) and the interior of the guide catheter (24) to a debris collection vessel (92).

    A device according to any one of the preceding Claims, wherein the infusate pump means (84) and the debris pump means (90) are each positive displacement pumps or peristaltic pumps.

    A device according to any one of the preceding Claims, including barrier means (126) for location downstream of the working head (32) and activatable to restrict the flow of the debris downstream of the barrier means (126).

    A device according to any one of the preceding Claims, wherein the working head (32) is arranged to rotate about its longitudinal axis to create a toroidal shaped vortex flow of the infusate liquid to reduce in size the particles broken off the inclusion by bringing them into repeated contact with impacting surfaces (54A, 54B)of the working head (32).

    A device according to any one of the preceding Claims, wherein the working head (32) can be arranged to be rotated at a speed to reduce the particles of the inclusion until 95% of the particles have a surface area less than that of a red blood cell.

    A device according to any one of the preceding Claims, further comprising a second guide catheter (124), the lumen opening catheter (22') having the working head (32') arranged to pass via the second guide catheter (124) into contact with the lumen to establish a fluid flow path between the second guide catheter (124) and the lumen opening catheter (22') and barrier means (126) to be disposed distally of the working head (32').
