Laser assisted atherectomy device and method.

Abstract

There are described a surgical catheter (10) and a method that removes stenosis and prevents restenosis. The catheter comprises a laser-emitting device (110), most preferably a laser diode, in the distal end (20), whereby optical fibers and their attendant disadvantages are eliminated. Preferably, the diode (110) is coupled with a mechanical cutting edge (56), so that the diode (110) can expose to low power laser light (130) freshly exposed, healthy luminal surfaces (S). Such exposure devitalizes and seals such surfaces to substantially prevent restenosis.

    A surgical catheter comprising a distal end constructed for movement within body vessels of a patient, a proximal end constructed to remain exterior of the patient to allow control of the distal end, a body portion connecting said two ends, said distal end including emitting means for emitting laser light at an energy and in a direction sufficient to treat the sidewalls of the body vessel, and means for generating said laser light;characterized in that said generating means and said emitting means comprise at least one laser diode located in said distal end, and at least one wire extending at least most of the length of said body portion to provide electrical energy to said at least one laser diode.

    A surgical catheter comprising a distal end constructed for movement within body vessels of a patient, a proximal end constructed to remain exterior of the patient to allow control of the distal end, a body portion connecting said two ends, said distal end including means for cutting away occlusive material from the interior walls of the patient body vessel, said cutting means including a metallic cutting edge and means for moving it relative to the plaque to be cut;characterized in that said distal end further includes, at a location adjacent to said cutting edge, means for emitting laser light out of the side of said distal end, said emitting means being mounted so as to expose the surface of the body vessel immediately following the passage of said cutting edge over that surface, so that said emitting means is effective to devitalize physiologically active tissues.

    A method for removal of stenosis in body vessels and the devitalization of physiologically active tissues, comprising the steps of a) inserting into an incision in the patient a surgical catheter comprising a distal end constructed for movement within body vessels of a patient, a proximal end constructed to remain exterior of the patient to allow control of the distal end, a body portion connecting said two ends, said distal end including emitting means for emitting laser light at an energy and in a direction sufficient to treat the sidewalls of the body vessel;b) removing stenosis at sites on the wall of the body vessel to expose physiologically active, healthy tissue, and c) immediately thereafter, treating said healthy, active tissue with said laser light to devitalize it.
