Trocar In veterinary medicine trocar is used for emptying the rumen of pressurized gas masses. In a known embodiment of this type, a sleeve provided with a central inner bore is provided which has at its one end a stop in the form of a plate. This plate settles in the operating position of the trocar against the outer surface of the abdominal wall of the cattle and the like. For mounting holes are incorporated in this plate through which the stitching books are guided in the outer skin.
The disadvantage is that these seams can anchor the plate only a few days to the skin. The deeper tissue layers, but especially in the free Bauehhdhie moving rumen wall slides freely along the trocar, so that it protrudes into the free abdominal cavity. These unsuccessful attempts have been made to repair trocar constructs with folding mechanisms at the end of the sleeve. However, these are unable to withstand the pulling force of the contracting rumen wall.
This is also the reason why so far only a surgical fistula fistula could be created, which requires an annular attachment of the rumen wall to the outer skin.
The object of the invention is to provide a trocar, with which an instrumental rumen fistula can be produced, which withstands the tensile and stress conditions of the rumen wall with respect to the abdominal wall. The invention is characterized by a subsequent to the shaft, down to tapering cutting part in the form of a spiral screw, the uppermost spiral passage is designed as a stop, which rests against the inner wall of the hollow organ.
In a further embodiment of the inventive concept, the distance between the two stops is approximately equal to the thickness of the pierced layers.
Suitably, the length of the spiral roach is greater than the thickness of the pierced layers.
The arrangement of the two abutments of trocar according to the invention in the abdominal and rumen wall receives a tight fit and simultaneously holds several mutually displaceable wall layers together. Its application also leads to an annular adhesion of the rumen wall with the abdominal wall. This adhesion ensures the completion of the free abdominal cavity to the outside.
The trocar according to the invention is introduced in cattle with chronic recurrent tympania at the usual trocar site on the left side of the hunger pits. For this purpose, a skin incision is made after disinfection and anesthesia Stichstellen a, which has the length of half the diameter of the inner stop. Then, after insertion of a stylet, the trocar is threaded through the outer skin, the tissue layers of the abdominal wall, and the rumen wall such that the spiral screw-shaped portion lies within the lumen of the rumen.
The trained as a stop last spiral passage presses the rumen wall flat against the inner baffle. The outer stop serves as an abutment. This leads to the trocar around quickly to an annular bonding and adhesion of the rumen wall to the inner abdominal wall. After removal of the stylet, the gas pressure of the rumen through the trocar sleeve can escape constantly to the outside. The overstretched cane winds can recur and regain their normal motility. The trocar according to the invention can be left in this position for as long as desired. It can be removed at any time by pulling out or by cutting off the outer stop.
An embodiment of the trocar according to the invention is explained below with reference to the drawing a little more detail. 1 shows a view of the trocar according to the invention; FIG. 2 shows a plan view of the arrangement according to FIG. 1. FIG. 3 shows a vertical section through the arrangement according to FIG. 1. FIG. 4 shows a view of the trocar according to the invention in the operating position.
1 with a preferably cylindrical shaft is referred to, which merges at its one end in a plate-shaped stopper 2.
According to a feature of the invention, the plate-shaped stop has approximately the shape of the outline of an egg. The special training has the advantage of easier handling of the trocar when screwing. It should be noted that when applying the trocar solid tissue layers must be cut, for which a certain amount of force is required.
Suitably, the plate-shaped stopper 2 has at its periphery a bead 2a. According to another feature, a channel-shaped recess 2b is incorporated in the plate-shaped stopper 2. The bead 2a of the stopper 2 causes the rumen liquid passing through the bore 4 to be guided to the tip of the stopper 2 and to run better through the channel-shaped recess 2b. The angling of one half of the stop 2 is determined by the tension and tension conditions which act on the trocar sleeve. This ensures a perfect fit at the trocar site.
According to the invention, a downwardly tapered cutting part in the form of a spiral screw 3 adjoins the lower part of the shaft 1, the uppermost spiral passage being designed as a stop 3a.
The upper spiral course of the spiral roughness 3 is rounded at its edges, while the lower spiral paths are sharp-edged (FIG. 3). In this case, the horizontal part of the uppermost spiral course 3a of the spiral screw 3 is approximately perpendicular to the shaft 1. The sharp-edged spiral courses are used for better cutting through the tissue layers; on the other hand, the uppermost spiral course must be rounded off, as it may be as an opposing retaining plate must serve for weeks and would cut in the case of a sharp-edged training the pulling rumen wall. For the same reason, the horizontal part of the uppermost screw thread 3a of the spiral screw 3 is approximately perpendicular to the shaft 1.
According to a further feature of the invention, the lower unthreaded end of the trocar is about 20 mm long, must lie close to the stylet and run out sharp-edged towards the tip.
The tip gives the trocar a better grip during handling and prevents the tissue from slipping off.
The beginning of the spiral screw 3 is below the angled part of the stop 2, so that a tight fit is ensured, which excludes slipping of the rumen wall (Fig. 2).
The trocar according to the invention further has a central bore 4, through which a stiletto is introduced and then the gases from the inner organ, for. B. from the bovine rumen, can escape to the outside.
In the operating position, the trocar according to the invention assumes the position shown in FIG. 4 of the drawing. After perforation of the abdominal wall 5 and the rumen wall 6 by insertion of the stylet through the bore 4 of the trocar, the spiral scraper 3 is screwed through the abdominal wall 5 and the rumen wall 6. The length L of the spiral screw 3 is greater than the thickness of the pierced layers. Further, the distance 1 between the two stops 2 and 3a is approximately equal to the thickness of the abdominal wall 5 and the rumen wall. 6
In the end position, the abdominal wall 5 and the rumen wall 6 are clamped in the vicinity of the trocar between these two stops 2 and 3a.
The thickness of the wall of the shaft 1, the flanks of the spiral screw 3 and the upper stop 2 depends on the thickness and strength of the layers to be pierced. These parts also determine the material of the trocar.
The trocar according to the invention is used in all disturbances of rumen activity of the bovine associated with excessive gas pressure conditions, in particular in the treatment of chronic recurrent tympania. It replaces the previously required rumen fistula surgery.
The trocar according to the invention can also be used for the introduction or
Removal of gaseous, liquid or solid materials in or out of a hollow body, be applied advantageously.
