Cardiac treatment system and method

Abstract

Devices and methods for providing localized pressure to a region of a patient's heart to improve heart functioning, including: (a) a jacket made of a flexible biocompatible material, the jacket having an open top end that is received around the heart and a bottom portion that is received around the apex of the heart; and (b) at least one inflatable bladder disposed on an interior surface of the jacket, the inflatable bladder having an inelastic outer surface positioned adjacent to the jacket and an elastic inner surface such that inflation of the bladder causes the bladder to deform substantially inwardly to exert localized pressure against a region of the heart.

  1. 1
A cardiac implant for implantation around an exterior of a heart, the implant comprising:an implant body comprising a mesh material that is formed into a generally tubular configuration, the implant body comprising a leading end portion defining an open leading end and a trailing end portion defining an open trailing end of a diameter that is less than a diameter of the open leading end, the trailing end portion having a length configured to extend proximal to the apex of the heart when the leading end portion is positioned in an atrial-ventricular groove of the heart;and a fillable bladder mounted to the implant body and positioned relative to the open leading end such that, when the leading end portion is positioned in the atrial-ventricular groove, the fillable bladder is positionable to exert localized pressure on an exterior of a heart wall adjacent to a valve of the heart, wherein the fillable bladder is configured to be sealed to maintain a selected inflation state and to expand inwardly toward the heart wall more so than outwardly when the fillable bladder is inflated.
  2. 5
The cardiac implant of any one of claims 1 to 4, wherein the fillable bladder comprises an inner bladder layer positioned with respect to the mesh material of the implant body to face inwardly toward the exterior of the heart wall when the leading end portion is positioned in the atrial-ventricular groove of the heart, and an outer bladder layer positioned opposite from the inner bladder layer, wherein the inner bladder layer has a compliance that is greater than a compliance of the outer bladder layer.
  3. 6
The cardiac implant of any one of claims 1 to 5, further comprising at least a first radiopaque marker positioned along a periphery of the fillable bladder, and at least a second radiopaque marker positioned along a periphery of the leading end portion proximate to the leading open end.
  4. 7
A cardiac implant for implantation around an exterior of a heart, the implant comprising:an implant body comprising a mesh material that is formed into a generally tubular configuration, the implant body defining an open leading end and an open trailing end of a lateral width that is less than a lateral width of the open leading end, the implant body comprising: a generally cylindrical leading end portion defining the open leading end of the implant body, wherein the leading end portion has a lateral width and construction for positioning around an atrial-ventricular groove of the heart, a transition portion extending proximally from the leading end portion and configured to circumferentially encompass ventricles of the heart, the transition portion having a greater diameter at its distal end than at its proximal end, and a generally cylindrical trailing end portion extending proximally from the transition portion and having a length sized such that the trailing end portion extends proximal to the apex of the heart when the leading end portion is positioned around the atrialventricular groove;and a fillable bladder coupled to the implant body and positioned such that, when the leading end portion is positioned around the atrial-ventricular groove, the fillable bladder is positionable to exert localized pressure on an exterior of a heart wall adjacent to a heart valve structure, □ate Reçue/Date Received 2021-10-04 wherein the fillable bladder is configured to be sealed to maintain a selected inflation state and to expand inwardly toward the heart wall more so than outwardly when the fillable bladder is inflated.
  5. 10
The cardiac implant of any one of claims 7 to 9, wherein the opening at the open trailing end of the implant body is sized to accommodate delivery device components removably attached to the leading end portion.
  6. 11
The cardiac implant of any one of claims 7 to 10, wherein the fillable bladder comprises an inner bladder layer positioned with respect to the mesh material of the implant body to face inwardly toward the exterior of the heart wall when the leading end portion is positioned around the atrial-ventricular groove of the heart, and an outer bladder layer positioned opposite from the inner bladder layer, wherein the inner bladder layer has a compliance that is greater than a compliance of the outer bladder layer.
  7. 12
The cardiac implant of any one of claims 7 to 11, further comprising at least a first radiopaque marker positioned along a periphery of the fillable bladder, and at least a second radiopaque marker positioned along a periphery of the leading end portion proximate to the leading open end. □ate Reçue/Date Received 2021-10-04
  8. 13
A cardiac implant, comprising:an implant body comprising a mesh material, the implant body comprising a leading end portion defining an open leading end and a trailing end portion defining an open trailing end of a diameter that is less than a diameter of the open leading end;and a fillable bladder coupled to the implant body, the fillable bladder positionable to exert localized pressure on an exterior of a heart wall adjacent to a valve of a heart, wherein the fillable bladder is configured to be sealed to maintain a selected inflation state and to expand inwardly toward the heart wall more so than outwardly when the fillable bladder is inflated.
  9. 16
The implant of any one of claims 13 to 15, further comprising at least a first radiopaque marker positioned along a periphery of the fillable bladder, and at least a second radiopaque marker positioned along a periphery of the leading end portion proximate to the leading open end.
