Device for delivery of antimicrobial agent into a trans-dermal catheter

Abstract

A system, method, and article for delivering an antimicrobial agent into the lumen of a trans-dermal catheter are disclosed. In an embodiment, the system comprises an elongate member configured for insertion into a lumen of a catheter, and the elongate member containing an antimicrobial. In the alternative an antimicrobial agent can be placed on an interior surface of a retaining ring.

    What is claimed is:1. A method of delivering an antimicrobial composition into a transdermal catheter, the method comprising: a) providing a transdermal catheter for implantation within a patient, the transdermal catheter having a proximal end for use outside of the patient and a distal end for use at least partially within a blood vessel of the patient, the catheter comprising: i) a hub located at the proximal end of the catheter, ii) exterior threads on the proximal end of the hub, and iii) an interior channel in the hub leading from an opening at the proximal end of the catheter to a lumen in the catheter, wherein at least a portion of the interior channel has a tapered interior surface;b) providing an antimicrobial composition delivery device for insertion into the proximal opening of the catheter, the antimicrobial composition delivery device comprising: i) a tapered member configured for insertion into the catheter hub, the tapered member configured to seal the proximal end of the catheter, ii) an elongate member extending from the tapered member, the elongate member configured for insertion into the catheter hub, iii) an antimicrobial composition positioned on the elongate member, and iv) a retaining ring comprising threads configured to engage the exterior threads on the catheter hub;c) injecting a liquid lock solution into the transdermal catheter such that at least the proximal end of the transdermal catheter is filled with the lock solution;d) applying a clamp across the proximal end of the catheter, the clamp preventing the flow of fluids across the clamped portion of the catheter;and e) after applying the clamp, insertion of the elongate member and tapered member of the antimicrobial delivery device into the hub located at the proximal end of the catheter;wherein the elongate member is retained within the hub of the transdermal catheter;wherein the elongate member is entirely proximal to the clamp;wherein the tapered member of the antimicrobial delivery device sealingly engages the tapered member of the CA 2841832 2018-09-05 hub of the catheter;and wherein the antimicrobial composition elutes into the lock solution on the proximal end of the clamp.

    The method of claim I, wherein the antimicrobial composition forms a precipitate that possesses antimicrobial properties.

    The method of claim I, wherein the elongate member is contained fully within the hub.

    The method of claim I, wherein the elongate member has a cross sectional area less than 50 percent of the smallest cross sectional area of the tapered member.

    A method of coating an antimicrobial composition on a transdermal catheter, the method comprising:a) providing a transdermal catheter for implantation within a patient, the transdermal catheter having a proximal end for use outside of the patient and a distal end for use at least partially within a blood vessel of the patient, the catheter comprising: i) a hub located at the proximal end of the catheter, ii) exterior threads on the proximal end of the hub;iii) an interior channel leading from an opening at the proximal end of the catheter to a lumen in the catheter, wherein at least a portion of the interior channel has a tapered interior surface;b) providing an antimicrobial delivery device for insertion into the proximal opening of the catheter, the device comprising: i) a tapered member configured for insertion into the catheter hub, the tapered member configured to seal the proximal end of the catheter, CA 2841832 2018-09-05 ii) an elongate member extending from the tapered member, the elongate member configured for insertion into the catheter hub, iii) an antimicrobial composition positioned on the antimicrobial delivery device, and iv) a retaining ring comprising threads configured to engage the exterior threads on the catheter hub;c) injecting a liquid lock solution into the transdermal catheter such that at least the proximal end of the transdermal catheter is filled with the lock solution;d) applying a clamp across the proximal end of the catheter, the clamp preventing the flow of fluids across the clamped portion of the catheter;and e) after applying the clamp, insertion of the elongate member and the tapered member of the antimicrobial delivery device into the hub located at the proximal end of the catheter;wherein upon insertion of the elongate member, the antimicrobial composition forms an antimicrobial precipitate within the lock solution;wherein the elongate member is entirely proximal to the clamp;and wherein the antimicrobial precipitate coats an internal channel of the catheter.

    A method of delivering an antimicrobial composition to a transdermal catheter, the method comprising:a) providing a transdermal catheter for implantation within a patient, the transdermal catheter having a proximal end for use outside of the patient and a distal end for use at least partially within a blood vessel of the patient, the catheter comprising: i) a hub located at the proximal end of the catheter, ii) exterior threads on the proximal end of the hub, and iii) an interior channel in the hub leading from an opening at the proximal end of the catheter to a lumen in the catheter, wherein at least a portion of the interior channel has a tapered interior surface;b) providing an antimicrobial composition delivery device for insertion into the proximal opening of the catheter, the antimicrobial composition delivery device comprising: i) a retaining ring comprising threads configured to engage the exterior threads on the catheter hub;ii) a soluble non-liquid antimicrobial composition located on the CA 2841832 2018-09-05 threads of the retaining ring;and iii) a tapered member configured for insertion into the catheter hub, the tapered member configured to seal the proximal end of the catheter;c) injecting a liquid lock solution into the transdermal catheter such that at least the proximal end of the transdermal catheter is filled with the lock solution;d) applying a clamp across the proximal end of the catheter, the clamp preventing the flow of fluids across the clamped portion of the catheter;and e) after applying the clamp, insertion of the tapered member of the antimicrobial delivery device into the hub located at the proximal end of the catheter so as to displace liquid lock solution from the interior of the catheter hub to the exterior of the catheter hub such that at least a portion of the antimicrobial composition on the threads of the retaining ring is transferred to the threads of the hub.

    The method of claim X, further comprising evaporation of at least a portion of the displaced liquid lock solution so as to form a deposit of non-dissolved antimicrobial composition on the threads of the retaining ring.

    A method of delivering an antimicrobial composition into a transdermal catheter, the method comprising; a) providing a transdermal catheter for implantation within a patient, the transdermal catheter having a proximal end for use outside of the patient and a distal end for use at least partially within a blood vessel of the patient, the catheter comprising:i) a hub located at the proximal end of the catheter, ii) exterior threads on the proximal end of the hub, and iii) an interior channel in the hub leading from an opening at the proximal end of the catheter to a lumen in the catheter, wherein at least a portion of the interior channel has a tapered interior surface;b) providing an antimicrobial composition delivery device for insertion into the proximal opening of the catheter, the antimicrobial composition delivery device comprising: i) a retaining ring comprising threads configured to engage the exterior threads on the catheter hub;ii) a soluble non-liquid antimicrobial composition located on the threads of the retaining ring;iii) a tapered member configured for insertion into the catheter hub, the tapered member configured to seal the proximal end of the catheter;and iv) an elongate member located on the antimicrobial deliver device, the elongate member configured for insertion into the hub of the transdermal catheter along with the tapered member;c) injecting a liquid lock solution into the transdermal catheter such that at least the proximal end of the transdermal catheter is filled with the lock solution;d) clamping the transdermal catheter;and e) insertion of the tapered member and elongate member of the antimicrobial delivery device into the hub located at the proximal end of the clamped catheter so as to displace the liquid lock solution from the interior of the catheter hub to the exterior of the catheter hub such that at least a portion of the antimicrobial composition on the threads of the retaining ring is transferred to the threads of the hub;wherein upon insenion of the elongate member into the hub, the elongate member CA 2841832 2018-09-05 displaces a portion of the liquid lock solution from the interior of the hub to the exterior of the hub.

    A method of delivering an antimicrobial composition into a Iransdermal catheter, the method comprising:a) providing a transdermal hemodialysis catheter for implantation within a patient, the transdermal hemodialysis catheter having a proximal end for use outside of the patient and a distal end for use at least partially within a blood vessel of the patient, the catheter comprising: i) a hub located at the proximal end of the catheter, ii) exterior threads on the proximal end of the hub, and iii) an interior channel in the hub leading from an opening at the proximal end of the catheter to a lumen in the catheter, wherein at least a portion of the interior channel has a tapered interior surface;b) providing an antimicrobial composition delivery device for insertion into the proximal opening of the catheter, the antimicrobial composition delivery device comprising: i) a retaining ring comprising threads configured to engage the exterior threads on the catheter hub;ii) a soluble chlorhexidine composition located on the threads of the retaining ring;and iii) a tapered member configured for insertion into the catheter hub, the tapered member configured to seal the proximal end of the catheter;and iv) an elongate member located on the antimicrobial deliver device, the elongate member configured for insertion into the hub of the transdermal catheter along with the tapered member;c) injecting a liquid lock solution into the transdermal catheter such that at least the proximal end of the transdermal catheter is filled with the lock solution;wherein injecting the liquid lock solution comprises: i) using a syringe to inject the liquid lock solution, the syringe partially inserted into the catheter hub;ii) clamping the transdermal catheter;iii) upon removing the syringe from the hub, the syringe tip leaves a unfilled void at the proximal end of the hub, and CA 2841832 2018-09-05 iv) wherein the tapered member and elongate member have a volume greater than the volume of the unfilled void;e) insertion of the tapered member and elongate member of the antimicrobial delivery device into the hub located at the proximal end of the clamped catheter so as to displace liquid lock solution from the interior of the catheter hub to the exterior of the catheter hub such that at least a portion of the chlorhexidine composition on the threads of the retaining ring is transferred to the threads of the hub;f) formation of a precipitate of the chlorhexidine composition in the hub of the catheter;g) evaporation of at least a portion of the displaced liquid lock solution so as to form a deposit of non-dissolved antimicrobial composition on the threads of the hub;and h) exposing the hub to moisture sufficient to redissolve at least a portion of the antimicrobial composition located on the threads of the hub.

    Λ device for insertion into a hub of a transdermal catheter to provide antimicrobial properties, the device comprising:a) a cap configured to be removably secured to a proximal end of a transdermal catheter, the cap comprising: i) an outer surface for handling the device, and ii) an inner volume having an interior surface, the interior surface containing threads configured to engage threads on the transdermal catheter;b) a tapered member positioned at least partially within the inner volume of the cap, such that the tapered member is located opposite the threads of the cap;and c) an elongate member secured to the tapered member and configured for insertion into the hub of the transdermal catheter, the elongate member comprising an antimicrobial agent;wherein the tapered member and elongate member are configured for insertion into the proximal end of the transdermal catheter such that antimicrobial agent from the elongate member is delivered into the hub of the transdermal catheter proximal to a clamp, the clamp on the proximal end of the transdermal catheter;and wherein the elongate member, upon insertion into the hub of the transdermal catheter, is configured to direct fluids (i) radially away from the elongate member, (ii) toward the threads on the transdermal catheter and (iii) not distal to the clamp.     A device for insertion into a hub of a transdermal catheter to provide antimicrobial properties, the device comprising:a) a cap configured to be removably secured to a proximal end of the transdermal catheter;b) a tapered member secured to the cap;and c) a rigid elongate member secured to the tapered member and configured for insertion into the hub of the transdermal catheter, the elongate member comprising an antimicrobial agent;wherein the tapered member and elongate member are configured for insertion into the proximal end of the transdermal catheter proximal to a clamp, the clamp on the proximal end of the transdermal catheter, such that antimicrobial agent from the elongate member is delivered into the hub of the transdermal catheter;and wherein upon insertion into the hub of the transdermal catheter, the elongate member is configured to direct fluids (i) radially away from the elongate member, (ii) toward the threads on the transdermal catheter and (iii) proximal to the clamp.

    A de vice for insertion into a hub of a transdermal catheter to provide antimicrobial properties, the device comprising:a cap configured to be removably secured to a proximal end of the transdermal catheter;an elongate member secured to the cap and configured for insertion into the hub of the transdermal catheter, the elongate member comprising an antimicrobial agent located along a portion of the length of the elongate member;wherein the elongate member is configured to be retained entirely proximal to a clamp on the proximal end of the transdermal catheter, and the elongate member is removable from the hub by removal of the cap;and wherein the elongate member is configured to direct fluids radially away from elongate member upon insertion into the hub of a transdermal catheter.     A device for insertion into a hub of a transdermal catheter to provide antimicrobial properties, the device comprising:a) a cap configured to be removably secured to a proximal end of the transdermal catheter, the cap comprising: i) an outer surface for handling the device, and ii) an inner volume having an interior surface, the interior surface containing threads configured to engage threads on the transdermal catheter, the interior surface threads comprising an antimicrobial agent;b) a tapered member positioned at least partially within the inner volume of the cap such that the tapered member is located opposite the threads of the cap, the tapered member having a taper of at least 5 percent;and c) a rigid elongate member secured to the tapered member and configured for insertion into the hub of the transdermal catheter, the elongate member comprising an antimicrobial agent;wherein the tapered member and elongate member are configured for insertion into the proximal end of the transdermal catheter, proximal to a clamp on the proximal end of the transdermal catheter, such that the antimicrobial agent from the elongate member is delivered into the hub of the transdermal catheter and to the threads of the transdermal catheter.
