Embolic deflection device and method of use

Abstract

There is disclosed a porous emboli deflector (100) for preventing cerebral emboli while maintaining cerebral blood flow during an endovascular or open surgical procedure. The device prevents the entrance of emboli of a size able to cause stroke (such as greater than 100 microns) from entering either the right (22) or left (24) common carotid arteries by deflecting emboli downstream of these vessels. The device (100) can be placed prior to any manipulation of the heart or aorta (10) allowing maximal protection of the brain during the index procedure. The deflector (100) has a low profile within the aorta (10) which allows sheaths, catheters, or wires used in the index procedure to pass. Also disclosed are methods for insertion and removal of the deflector (100).

A temporary emboli diversion device, comprising:an elongate, flexible shaft, having a proximal end and a distal end;and a deflector on the distal end;the deflector comprising a flexible frame having a length extending between a first end and a second end and a width extending along a midplane between a first side and a second side and a membrane attached thereto, the deflector connected to the flexible shaft by first and second struts which diverge radially outwardly in the distal direction;wherein the deflector is convertible between a folded configuration in which both the first end and the second end point in the distal direction, and a deployed configuration in which the first and second ends point in lateral directions, and wherein the deflector frame includes first and second lobes, each formed of a thin curved member having opposite medial ends each of which is connected to one of the first and second struts, and extends in opposite lateral directions from the mid-plane to the first and second ends so as to form a complete loop with the first and second ends farthest away from the mid-plane, wherein the divergent first and second struts further laterally diverge in opposite lateral directions and are contiguous with the loops defined by the first and second lobes, wherein the deflector is concave in a proximal direction and convex in a distal direction, and the combination of the first and second lobes defines a periphery of an oval shape having a minor axis on the midplane and a major axis perpendicular thereto.

    The device of Claim 1, wherein the first end and the second end comprise radiopaque markers.

    The device of Claim 1, further comprising:an elongate flexible tubular body, having a proximal end, a distal end, and at least one lumen extending therethrough;-33CA 2749385 2017-04-25 the elongate, flexible shaft, axially movably extending through the lumen;and the deflector movable between the folded configuration being a first configuration for positioning within the lumen and the deployed configuration being a second configuration for deployment;wherein the deflector in the second configuration comprises a length measured transverse to the shaft which exceeds a width measured perpendicular to the length.

    The device of Claim 3, wherein the length is at least 150% of the width.

    The device of Claim 3, wherein the length is at least 200% of the width.

    The device of Claim 3, wherein the length is at least 250% of the width.

    The device of Claim 3, wherein the deflector comprises a suture loop encircling a portion of the flexible frame in at least one location on the periphery of the deflector.

    The device of Claim 1, wherein the deflector has only a single plane of symmetry and the shaft lies within the plane of symmetry.

    The device of Claim 1, wherein the membrane has a pore size of no greater than 100 micrometers.

    The device of Claim 3 wherein the deflector extends transversely with respect to the shaft between the first end and the second end;and the device further comprises a first radiopaque marker carried by the first end and a second radiopaque marker carried by the second end.

    The device of Claim 3 further comprising a torque control carried by the shaft.

    The device of Claim 1, wherein the deflector is curved in at least two axes such that it lacks radial symmetry with respect to a longitudinal axis of the shaft, and a peripheral edge of the deflector has a three dimensional configuration such that it conforms approximately to an interior surface of a non spherical geometry of rotation when the -34CA 2749385 2017-04-25 deflector is positioned in a main vessel and when the shaft extends through a branch vessel under traction.

    The device of Claim 12, wherein the geometry of rotation is a cylinder.
