Devices and methods for bone anchoring

Abstract

Embodiments of the invention relate generally to tissue anchors and methods of delivering same to the intervertebral disc or other sites within the body. In some embodiments, the anchors provide pull-out resistance, stability and/or maximize contact with tissue involving a minimum amount of penetration. In some embodiments, delivery methods are minimally invasive and include linear, lateral, and off-angle implantation or driving of anchors along, against or within tissue surfaces.

    CA 02635537 2013-08-12 THE SUBJECT-MATTER OF THE INVENTION FOR WHICH AN EXCLUSIVE PROPERTY OR PRIVILEGE IS CLAIMED IS DEFINED AS FOLLOWS:1. A bone anchor system for insertion into a first surface of a bone and along an adjacent second surface of said bone that is offset at an angle substantially perpendicular from the first surface, comprising: an elongated keel portion having a length defined by a sharpened leading edge and a trailing end;a neck extending from an upper surface of the keel portion and having a length defined by a sharpened leading edge and a trailing end;wherein said neck comprises an attachment site along at least a portion of its length;wherein the neck length is shorter than the length of the keel portion;wherein the sharpened leading edge of the keel portion is adapted to be driven into the first surface of the bone along a substantially straight trajectory path that is substantially parallel to the second surface while simultaneously advancing the attachment site across said second surface;and wherein, in use, the attachment site is coupled to tissue or to a prosthetic implant for repairing said bone or adjacent tissue.

    The bone anchor system of Claim 1, wherein said keel portion comprises two keels, and further wherein the two keels form an angle from about 10 to about 180 degrees relative to each other.

    The bone anchor system of Claim 1, wherein said keel portion comprises two keels, and further wherein the two keels form an angle of 180 degrees relative to each other.

    The bone anchor system of Claim 1, wherein said keel portion is an elongated platelike keel portion.

    The bone anchor system of Claim 1, wherein the attachment site is coupled to a prosthetic implant that comprises at least one of a mesh, barrier or graft.

    The bone anchor system according to any one of Claims 1-6, wherein a prosthetic implant is coupled to the neck of the bone anchor via the attachment site prior to insertion of the bone anchor.

    The bone anchor system according to any one of Claims 1-7, wherein at least a portion of the anchor is coated with a biologically active or therapeutic agent.

    The bone anchor system according to any one of Claims 1-8, wherein a prosthetic implant is coupled to the attachment site of the bone anchor prior to insertion;wherein the prosthetic implant is rotationally coupled to the bone anchor via the attachment site to facilitate low-profile insertion of the bone anchor;wherein the prosthetic implant comprises a polymer;and wherein the keel portion comprises titanium.

    The bone anchor system according to any one of Claims 1-9, wherein the attachment site comprises a mesh, fabric, or membrane material, or a combination thereof.

    The bone anchor system according to any one of Claims 1-10, wherein at least a portion of said neck or keel portion comprises scalloped ridges to facilitate tissue growth.

    The bone anchor system according to any one of Claims 1-11, further comprising ribs that protrude along portions of said keel portion to provide enhanced engagement with said bone or a tissue adjacent said bone. CA 02635537 2013-08-12

    The bone anchor system according to any one of Claims 1-12, wherein said anchor further comprises barbs, tabs, roughened surface geometry, polished surfaces, coatings, seeded carrier or drug eluting coatings or elements, concavities, scalloped ridges, grooves, feet, ridges, voids, slots, ingrowth openings, secondary edges or ribs.

    The bone anchor system according to any one of Claims 1-13, wherein the attachment site comprises at least one of a head, flange, plate, disc, protrusion, channel, hole, cleat or eye.

    The bone anchor system according to any one of Claims 1-14, wherein the attachment site is offset relative to both the anchor’s angle of insertion and said neck to present said attachment site along the second surface of said bone while said keel portion is inserted into said first surface and wherein said keel portion is configured for pull-out resistance and stability by presenting a larger surface area below or embedded within said second surface of the bone relative to said neck.

    The bone anchor system according to any one of Claims 1-15, wherein the attachment site is adapted to accept or connect to at least one of a suture, linkage element, or threaded screw.

    The bone anchor system according to any one of Claims 1-16, wherein the bone anchor comprises a cross-section having a wedge, “V”, “U”, “T”, “W”, or “X” shape.

    The bone anchor system according to any one of Claims 1-17, wherein the anchor is at least partially constructed from a material selected from the group consisting of one or more of the following:nickel titanium alloy, titanium, cobalt chrome alloys, and steel.

    The bone anchor system according to any one of Claims 1-18, wherein the first surface of the bone is the exterior of a vertebral body and the second surface of the bone is the CA 02635537 2013-08-12 corresponding adjacent vertebral endplate, and wherein the attachment site is coupled to a prosthetic implant that is a prosthetic barrier.

    A bone anchor system for insertion into a first surface of a bone and along an adjacent second surface of said bone that is offset at an angle substantially perpendicular from the first surface, comprising:a neck comprising a sharpened leading edge and a trailing end, wherein said neck comprises an attachment portion, wherein said trailing end is configured to be countersunk within or left flush with the first surface of the bone;at least one sharpened keel, wherein said keel is configured for pull-out resistance and stability by presenting a larger surface area below or embedded within said second surface of the bone relative to said neck;wherein the attachment portion is positioned so as to present said attachment portion along the second surface of said bone while said keel is inserted into said first surface;wherein the sharpened keel is adapted to be driven into the first surface of the bone along a substantially straight trajectory path that is substantially parallel to the second surface while simultaneously advancing the attachment portion across said second surface;and wherein the attachment portion is coupled or configured to couple to a tissue or a prosthetic implant for repairing said bone or adjacent tissue.

    The bone anchor system of Claim 20, wherein the attachment portion is coupled to a prosthetic implant prior to insertion, and wherein the implant is rotationally coupled to the attachment portion.

    A bone anchor system for insertion into an outer surface and an endplate of a vertebral body, comprising:a bone anchor comprising a neck having a length defined by a sharpened leading edge and a trailing end, wherein said neck comprises an attachment site along at least a portion of its length, CA 02635537 2013-08-12 wherein the attachment site is operable to couple to a prosthetic device, wherein said neck comprises a bottom portion terminating in two keels, wherein said keels are configured for pull-out resistance and stability by presenting a larger surface area below the endplate and embedded in the outer surface of the vertebral body, wherein the outer surface is offset at an angle substantially perpendicular from the endplate and extends around a circumference of the vertebral body, wherein said keels form an angle of about 180 degrees relative to each other, wherein the neck and the keels form a “T” shape, wherein each of said keels comprises sharpened leading edges, wherein the sharpened leading edges of said keels are adapted to be driven into the outer surface of the vertebral body along a substantially straight trajectory path that is substantially parallel to the endplate while the attachment site is simultaneously advanced along and across the endplate;and wherein said trailing end is adapted to be recessed within said outer surface such that no portion of the anchor extends beyond said outer surface.

    The bone anchor system of Claim 22 wherein the prosthetic device is coupled to the bone anchor prior to insertion.

    The bone anchor system according to Claim 22 or 23, wherein the prosthetic device is rotationally coupled to the bone anchor.

    The bone anchor system according to any one of Claims 22-24, wherein the prosthetic device is configured for closing a defect in the endplate.
