Bioabsorbable polymeric implants and a method of using the same to create occlusions

Abstract

An embolic, bioabsorbable polymeric material (BPM) is incorporated into a coil to improve long-term anatomic results in the endovascular treatment of intracranial aneurysms. The material includes at least one biocompatible and bioabsorbable polymer and growth factors, is carried by hybrid bioactive coils and is used to accelerate histopathologic transformation of unorganized clot into fibrous connective tissue in aneurysms. An endovascular cellular manipulation and inflammatory response are elicited from implantation in a vascular location. Thrombogenicity of the biocompatible and bioabsorbable polymer is controlled by the composition of or proportioning the ratio of constituents marking up the polymer. The biocompatible and bioabsorbable polymer is at least one polymer selected for the group consisting of polyglycolic acid, poly-glycolic acid/poly-L-lactic acid copolymers, polycaprolactive, polyhydroxbutyrate/hydroxyvalerate copolymers, poly-L-lactide, polydioxanone, polycarbonates, and polyanhydrides.

    CA 02437694 2011-04-01 CLAIMS:1. An apparatus for use in occluding a vascular aneurysm comprising: a separable coil implant disposable into the aneurysm, which coil implant is retained within the aneurysm;and an endovascular catheter to dispose the implant into the aneurysm, wherein the implant comprises an inflammatory substance adapted to provoke a selected degree of controlled scar tissue formation in the aneurysm to occlude substantially the entire aneurysm without formation of excess scar tissue.

    The apparatus of any one of claims 2 to 7, wherein the constituents of the substance comprise more than one biocompatible polymer that is a polyglycolic acid, poly-glycolic acid/poly-L-lactic acid copolymer, polycaprolactive, polyhydroxybutyrate/hydroxyvalerate copolymer, poly-L-Iactide, polydioxanone, polycarbonate, polyanhydride, or a copolymer thereof, and a ratio of biocompatible polymers is selected so as to control the degree of scar tissue formation by modulating an inflammatory response to the implant.

    The apparatus of any one of claims 1 to 8, wherein the substance is adapted to provoke a selected degree of controlled scar tissue formation without provoking an intense chronic foreign body reaction.

    The apparatus of any one of claims 1 to 9, where the implant is bioabsorbable.

    The apparatus of any one of claims 1 to 10, where the implant is adapted to stimulate mild to strong cellular infiltration and proliferation in a process of degradation to accelerate fibrosis within aneurysms.

    The apparatus of any one of claims 1 to 12, wherein the implant is adapted to stimulate CA 02437694 2011-04-01 connective tissue growth.

    The apparatus of any one of claims 1 to 13, wherein the implant is adapted to restrict compaction of the implant.

    The apparatus of any one of claims 1 to 14, wherein the implant is adapted to restrict recanalization of the aneurysm.

    The apparatus of any one of claims 1 to 15, wherein the implant is for use to:induce organized connective tissue growth to fill the aneurysm;to retract the aneurysm over time by maturation of collagen fibers;to reduce aneurysm size over time;or to decrease aneurysm compression on brain parenchyma or cranial nerves.

    The apparatus of any one of claims 1 to 15, wherein the implant is for use to occlude the aneurysm with less thrombogenicity than metal coils and to accelerate aneurysm healing with less thrombogenicity than metal coils.

    The apparatus of any one of claims 1 to 17, wherein the implant is adapted to retain the substance so that no residua of the substance is retained in the aneurysm.
