Apparatus and method for implanting prostheses within periurethral tissues

Abstract

An apparatus is disclosed for guiding a medical instrument to apredetermined target location within the periurethral tissues of a patient. Theapparatus directs the instrument along a predetermined path with respect to thepatient's urethra (250) and limits penetration of the instrument to a predetermineddepth. The disclosed embodiment comprises an apparatus for implanting inflatableprostheses (220) within the periurethral tissues to coapt the urethra (250) andthereby manage incontinence. A method for effecting coaptation of a urethra(250) of a patient is also disclosed. According to this method, a pair of workingchannels are formed within the periurethral tissues, and an inflatable prosthesis(220) is introduced into each working channel. The prostheses (220) are inflatedonly after both working channels have been formed, and preferably after bothprostheses (220) have been positioned within their respective working channels.The inflated prostheses (220) confront the urethra (250) and effect coaptation ofthe urethra (250).

An apparatus for implanting a periurethral prosthesis, the apparatus comprising: a template having a guide tube that is adapted to receive a positioning catheter, and at least one guide sleeve that is adapted to receive the implanting apparatus, the at least one guide sleeve including a guide sleeve stop;the positioning catheter being adapted to be inserted through the template guide tube and through a patient urethra, the positioning catheter including a shaft and an inflatable balloon that is positioned at a distal end of the shaft, the balloon being adapted to be inflated within a patient bladder adjacent a neck of the bladder, the positioning catheter further comprising a positioning catheter stop that is adapted to limit travel of the positioning catheter relative to the template guide tube and therefore limit insertion of the positioning catheter through the patient urethra;and means associated with the template for bearing against the patient so as to exert tension on the positioning catheter when the balloon of the catheter has been inflated within the patient bladder adjacent the bladder neck such that the position of the guide sleeve stop is fixed relative to the patient bladder neck.

    An apparatus for implanting a periurethral prosthesis, the apparatus comprising:a template having a central guide tube that is adapted to receive a positioning catheter and two outer guide sleeves that are aligned parallel to the central guide tube, the outer guide sleeves each including a stop and being adapted to receive implanting apparatus;the positioning catheter being adapted to be inserted through the central guide tube and through a patient urethra, the positioning catheter including a shaft and an inflatable balloon that is positioned at a distal end of the shaft, the balloon being adapted to be inflated within a patient bladder adjacent a neck of the bladder, the positioning catheter further comprising a positioning catheter stop that is adapted to limit travel of the positioning catheter relative to the central guide tube and therefore limit insertion of the catheter shaft through the patient urethra;means associated with the template for bearing against the patient so as to exert tension on the positioning catheter when the balloon of the catheter has been inflated within the patient bladder adjacent the bladder neck such that positions of the guide sleeve stops are fixed relative to the patient bladder neck;a trocar assembly that is adapted to be inserted through one of the outer guide sleeves and through periurethral tissues, the trocar assembly including a trocar assembly stop that limits the depth of insertion of a punch of the assembly relative to said one of the outer guide sleeves and therefore the bladder neck;and an implant apparatus that is adapted to be inserted through one of the outer guide sleeves after the trocar punch has been withdrawn from the periurethral tissues, the implanting apparatus comprising a cannula and an inflatable implant that is attached to a distal end of the cannula, wherein travel of the implanting apparatus through said one of the outer guide sleeves is limited by the respective guide sleeve stop such that the depth of insertion of the cannula and inflatable implant are controlled relative to the position of the bladder neck.
