Anti-infection and antithrombogenic medical articles and method for their preparation

Abstract

An anti-infective medical article has chlorhexidine bulk distributed throughout a polyurethane base layer and may have a coating layer on the base layer. The coating layer may be chlorhexidine permeated into the surface or it may be an antibiotic, antithrombogenic agent or a polymeric surface layer laminated onto the base layer. The invention includes a method for preparing the article wherein a homogeneous melt of polymer and chlorhexidine is prepared by twin screw compounding and the melt is extruded to give a medical article having bulk distributed chlorhexidine.

    - 27 WHAT IS CLAIMED IS:1. A method for preparing a medical article comprising: a) preparing a blend of chlorhexidine and pellets of a substantially hydrophilic polymer;b) heating said blend to a sufficient temperature and for a sufficient time to form a homogeneous melt of said polymer having said chlorhexidine uniformly distributed therein;and c) extruding said melt through a die to form a medical article having said chlorhexidine bulk distributed throughout said polymer.

    The method of Claim 1 further comprising applying to a surface of said article a coating of an agent selected from the group consisting of a second increment of chlorhexidine, a surface active agent and an antithrombogenic agent.

    The method in accordance with Claim 1 wherein said chlorhexidine is selected from the group consisting of chlorhexidine base and a salt thereof.

    The method in accordance with Claim 3 wherein said salt of chlorhexidine is selected from the group consisting of the hydrochloride, acetate and gluconate.

    The method in accordance with Claim 1 wherein said polymer is selected from the group consisting of polyurethane, polyurethaneurea and siloxane-urethane block copolymer.

    The method in accordance with Claim 5 wherein - 28 said polymer absorbs at least 0.6% of water.

    The method in accordance with Claim 1 wherein said chlorhexidine is about 0.1 to 10% by weight of said polymer.

    The method in accordance with Claim 2 wherein said applying step is performed by steeping said article in a solution of said agent.

    The method in accordance with Claim 2 further comprising ionically bonding an agent selected from the group consisting of an antibiotic and an antithrombogenic agent to said surface active agent.

    The method in accordance with Claim 1 wherein said melt is formed by twin screw compounding.

    The method in accordance with Claim 1 further comprising wetting said pellets with a polyol prior to said preparing.

    A method for preparing a medical article comprising preparing a homogeneous melt of a substantially hydrophilic polymer and an anti-infective agent and extruding said melt through a die to form a medical article having said agent bulk distributed throughout said polymer.

    A method for preparing a medical article comprising:a) preparing a blend of pellets of a substantially hydrophilic polymer containing urethane groups and about 1 to 10% by weight of a first - 29 increment of a salt of chlorhexidine;b) forming a homogeneous melt by twin screw compounding said blend at a temperature of at least 160°C;c) extruding said melt through a die to form a medical article having said chlorhexidine salt bulk distributed throughout said polymer;and d) steeping said medical article in a solution of an agent selected from the group consisting of a surface active agent and a second increment of a chlorhexidine salt to form a coating on said medical article.

    The method in accordance with Claim 13 further comprising ionically bonding an agent selected from the group consisting of an antibiotic and an antithrombogenic agent to said surface active agent.

    A method for preparing a medical article comprising:a) preparing a blend of chlorhexidine and a substantially hydrophilic base polymer;b) forming a homogeneous first melt from said blend;c) preparing a second melt of a substantially hydrophilic laminating polymer;d) coextruding said first and second melts through a die to form a medical article having a coating of said laminating polymer on said base polymer, said base polymer having chlorhexidine bulk distributed throughout.

    The method of Claim 15 wherein said second melt contains a second increment of chlorhexidine - 30 whereby said medical article additionally has said second increment of chlorhexidine bulk distributed throughout said laminating polymer.

    A medical article comprising a substantially hydrophilic polymeric base layer and a coating thereon, said base layer having a first increment of chlorhexidine bulk distributed throughout and said coating being selected from the group consisting of a second increment of chlorhexidine, an antibiotic and an antithrombogenic agent.

    The article of Claim 17 wherein said first and second increments of chlorhexidine are selected from the group consisting of chlorhexidine base and a chlorhexidine salt.

    The article of Claim 18 wherein said salt of chlorhexidine is selected from the group consisting of the hydrochloride, acetate and gluconate.

    The article of Claim 17 wherein said base layer is selected from the group of polymers consisting of polyurethane, siloxane-urethane block copolymer and polyurethaneurea.

    The article of Claim 20 wherein said polymer absorbs at least 0.6% by weight of water.
