Abstract

INSPIRATION OXYGEN SAVER Abstract of the Disclosure A reliable, pulse-flow supplementaloxygen apparatus for alleviating respiratoryailments is provided which yields substantialsavings in oxygen while giving the patient thephysiological equivalent of a prescribed continu-ous stream of oxygen. The apparatus preferablyincludes a demand oxygen valve operated in a pulsemode by means of electronic control circuitrywhich, through an appropriate sensor, monitors thepatient's breathing efforts and gives a variable"custom tailored" pulse volume of oxygen to thepatient during the very initial stages of eachinspiration. Pulse volume variability is basedupon a measured parameter characterizing at leasta part of one and preferably a plurality of thepatient's preceeding breaths; advantageously, theelapsed time interval of the patient's threepreceding breath cycles is measured to effectivelymeasure breath rate. These breath-characterizingparameters, together with data characterizing theprescribed continuous oxygen flow to be matched,enable the apparatus to give the desired "on thego" dose variability. The apparatus is alsodesigned to revert to conventional, continuous-flow operation upon a power failure or circuitmalfunction, and for thus purpose a specializeddual flow control restrictor valve is provided inthe form of the invention designed for hospitaluse. In the event of abnormally low or highbreath rates on the part of the patient, thedevice automatically delivers a relatively longcontinuous pulse, then stops to again sample thebreath rate. If the rate is normal, the usualpulse flow operation is resumed; if the rateremains abnormal, the long pulse volume-ratesampling is repeated. Actual tests with theapparatus hereof demonstrate an oxygen saving ofover 50% as compared with continuous flow regi-mens.

    The embodiments of the invention in which an exclusive right or privilege is claimed are defined as follows:1. Apparatus adapted for connection between a source of medicinal gas and a patient-coupled gas delivery device for supplying pulse volumes of medicinal gas from time to time during the patient's breathing cycles, said apparatus comprising: selectively actuatable valve means adapted for operable coupling between said gas source and device for selectively establishing and interrupting gas flow communication therebetween;and means operably coupled with said valve means for selectively actuating the same including breathing cycle sensing and measuring means for tracking the patient's breathing efforts over plural breathing cycles, and for measuring a variable magnitude parameter incident to at least one reference breathing cycle, said variable magnitude parameter being at least partially predictive of the volume of said gas required by said patient during a medicinal gassupplemented breathing cycle;means operably coupled with said sensing and measuring means for determining a variable magnitude value correlated with said measured parameter and representative of said required volume of medicinal gas;and means coupling said value-determining means and said valve means for opening said valve means during said medicinal gas-supplemented breathing cycle for delivering to said patient said required volume of medicinal gas, said volume being variable in response to the magnitude of said determined value for supplying to said patient variable volumes of said gas during different gas-supplemented breathing cycles.

    A fluid flow restricting device, comprising:a flow selector element presenting a pair of opposed faces and two sets of spaced apart orifices therethrough, the orifices of each set being of different effective dimensions respectively;and a body disposed about said element and having a wall surface adjacent each of said element faces, said body having first and second separate fluid inlet passageways therethrough and passing through one of said body wall surfaces, said body also having corresponding first and second, separate, fluid outlet passageways therethrough and passing through the other of said body wall surfaces, said first inlet passageway and said corresponding first outlet passageway, and said second inlet passageway and said corresponding second outlet passageway, being arranged for fluid flow communication therebetween, the individual orifices of each set thereof being oriented in mated pairs for presenting a 120 plurality of orifice pairs each including one orifice from each set thereof, means mounting said element within said body for selective, sequential, noncontinuous relative shifting movement between the element and body, and for selective stopping of said shifting movement at individual relative operating positions between the element and body, each of said orifice pairs being located and arranged for correlation with a respective one of said operating positions so that, for each of said operating positions, the orifice pair in correlation therewith establishes simultaneous fluid flow communication between said corresponding inlet and outlet passageways.

    A pulse translation apparatus for translating a series of input signals occurring at variable time intervals into a series of corresponding output signals such that each of the output signals commences in predetermined time relationship to the occurrence of a corresponding input signal and represents a time duration correlated in predetermined manner with the time intervals corresponding to a predetermined plurality of preceding input signals, said apparatus comprising:input means for receiving said input signals;output means for delivering said output signals;a plurality of timer means greater in number than said predetermined plurality of preceding input signals, and each operable for measuring a time 121 interval between events characterizing the occurrence of the first and last signals of said predetermined plurality of preceding input signals, and for generating an output signal representative of a time duration correlated in predetermined manner with said time interval;means for sequentially coupling each of said timer means with said input means in response to a successive one of said input signals;and means for sequentially decoupling each of said timer means from said input means, and for sequentially coupling each of said timer means with said output means in response to the last signal of said predetermined plurality of input signals.

    In an apparatus for delivering medicinal gas to a patient, said apparatus having means for tracking the breathing efforts of the patient and for producing a plurality of breath cycle signals corresponding to a predetermined event in each of a plurality of patient breathing cycles, a method of using said apparatus for 122 determining the breathing rate of the patient comprising the steps of:(a) starting one of a plurality of timers upon the occurrence of a first one of a plurality of said breath cycle signals;(b) stopping said one time upon the occurrence of a subsequent breath cycle signal, said subsequent breath cycle signal being at least the second signal after said first signal, (c) producing an output signal representative of the time interval between said first one and said subsequent breathing cycle signals, said output signal being correlated with and representative of the patient's breathing rate, (d) continuously repeating steps (a), (b) and (c) for each of said plurality of timers upon the occurrence of successive breath cycle signals respectively.

    An apparatus for translating a series of input signals occurring at variable time intervals into a series of corresponding output signals wherein the series of input signals presents a plurality of sets of input signals with each set having a predetermined plurality of input signals included therein, and in which each output signal is associated with an input signal set and represents the time interval between the first and last input signals of an associated set, said apparatus comprising:123 a plurality of selectively activatable and deactivatable timer means greater in number than said predetermined plurality, each of said timer means including means for producing an output signal reprssentative of the time interval between activation and deactivation thereof;and input means operably coupled with said timer means for receiving the input signals and for activating each timer means in response to the first input signal of the s t respectively associated therewith, and for deactivating each timer means in response to the last input signal of the set respectively associated therewith whereby each of said timer means produces a respective output signal representative of the time interval between the first and last input signal of an associated set of input signals and thereby representative of an average of variable time intervals between input signals included in the associated set.
