Multi-axial anchor assemblies for spinal implants and methods

Abstract

Systems and methods are provided that include a plate member (120, 140, 160, 180, 600) engageable to the spinal column with an anchor assembly (20, 320, 420, 520, 620, 720). The anchor assembly (20, 320, 420, 520, 620, 720) includes a coupling member (30, 130, 330, 430, 530, 630) having a post (32, 332, 432, 532, 632, 732) extending through at least one opening of the plate member (120, 140, 160, 180, 600) and an anchor member (70) pivotally captured in a receiver portion (34, 334, 434, 534, 634) of the coupling member (30, 130, 330, 430, 530, 630) below a lower surface of the plate member (120, 140, 160, 180, 600). A locking member (90, 590) secures the plate member (120, 140, 160, 180, 600) to the coupling member (30, 130, 330, 430, 530, 630). A crown (50, 150, 250, 350, 550, 650) in the receiver portion (34, 334, 434, 534, 634) of the coupling member (30, 130, 330, 430, 530, 630) is engaged by the plate member (120, 140, 160, 180, 600) to secure the anchor member (70) in position relative to the coupling member (30, 130, 330, 430, 530, 630).

    What is claimed is:1. An anchor assembly, comprising: an anchor member including a head and a lower portion extending from the head for engagement with a bone member;a coupling member pivotally coupled to said head of said anchor, said coupling member including a lower receiver portion defining an interior receptacle for receiving said head and a post extending from said lower receiver portion away from said head, said post being configured to engage a locking member, said lower receiver portion defining at least one sidewall opening in communication with an exterior of said coupling member;and a crown positioned in said receptacle of said coupling member about said head of said anchor member, said crown including a seat portion extending through said at least one sidewall opening for positioning the seat portion in contact with an implant member positioned about said post of said coupling member to secure the implant member between said locking member and said seating portion.

    A spinal plating system, comprising:an elongate plate member including at least one opening extending therethrough between an upper surface and an opposite lower surface, said lower surface being positionable along the spinal column;an anchor assembly engageable to said plate member, said anchor assembly including: 2005294535 03 Jan 2012 a coupling member having a post positionable through said at least one opening and a receiver portion positionable adjacent said lower surface of said plate member, said receiver portion including a receptacle;an anchor member including a head pivotally captured in said receptacle of said receiver portion and a lower portion extending from said head for engaging a bony structure of the spinal column;a crown positioned adjacent said head in said receptacle, said crown including a seat portion extending through at least one opening in said coupling member to the exterior of the coupling member and along said lower surface of said plate member;and a locking member engageable to said post and positionable in contact with said upper surface of said plate member to secure said lower surface of said plate member in contact with said seat portion of said crown.

    The assembly of any one of claims 1 to 3, 6 to 8, or the system of any one of claims 4 to 8, wherein said head includes a plurality of ridges extending thereabout adjacent said crown.

    The assembly of any one of claims 1 to 3, 6 to 12, or the system of any one of claims 4 to 12, wherein said post includes external threads extending therealong for threadingly receiving said locking member thereabout.

    The system of any one of claims 4 to 14, wherein said at least one opening of said plate member includes an elongate slot.

    The system of any one of claims 4 to 16, wherein:said plate member includes an arcuate profile along said upper and lower surfaces;and/or said plate member includes a plurality of slots therealong.

    A spinal surgical method, comprising:accessing at least one vertebra of the spinal column;engaging an anchor member of an anchor assembly to the at least one vertebra, the anchor assembly including a coupling member with a receiver portion pivotally mounted about a head of the anchor member and a post extending proximally from the receiver portion;moving the post by pivoting the receiver portion about the head of the anchor member to a desired position;positioning an elongate plate member about a proximal end of the post;advancing the plate member along the post to a location adjacent the receiver portion;and engaging the plate member against a crown exteriorly of the coupling member, the crown extending from a position adjacent the head of the anchor member, through at least one opening in the coupling member to the exterior of the coupling member, for engagement with the plate member.

    An anchor assembly, comprising:an anchor member including a proximal end and distal portion extending from said proximal end for engagement with a bone member;2005294535 03 Jan 2012 a coupling member pivotally coupled to said proximal end of said anchor, said coupling member extending along a longitudinal axis and including a lower receiver portion defining an interior receptacle extending about said axis for receiving said proximal end and a post extending from said receiver portion along said axis away from said proximal end, said coupling member defining at least one axially extending window in communication with an exterior of said coupling member;a member engageable to said post;and a crown positioned in said receptacle of said coupling member adjacent said proximal end of said anchor member, said crown including an axially extending seating portion extending through said at least one window for positioning in contact with said member engaged to said post of said coupling member.

    The assembly of any one of claims 21 to 25, wherein:said post is smooth along a substantial portion of a length thereof;and/or 2005294535 03 Jan 2012 said post includes a thread profile adjacent a distal end thereof, and said member is a locking member threadingly engageable with said thread profile in contact with said seating portion.

    The assembly of any one of claims 21 to 26, wherein said crown includes a distally opening cup portion extending from said seat portion, said cup portion including a receptacle for receiving said proximal end of said anchor member therein.

    The assembly of any one of claims 21 to 27, wherein:said post includes a circular cross-section and external threads thereabout for threadingly engaging said member;or wherein said post includes a distal engaging portion extending radially about a distal end of a threaded post portion, said engaging portion being threadingly engageable with a proximal end opening of said lower receiver portion.

    A spinal implant system, comprising:an implant including at least one opening extending therethrough between an upper surface and an opposite lower surface, said lower surface being positionable toward the spinal column;an anchor assembly engageable to said implant, said anchor assembly including: a coupling member having a post positionable through said at least one opening and a receiver portion positionable along said lower surface of said implant, said receiver portion including a receptacle and at least one proximally opening window in communication with said receptacle;an anchor member including a head pivotally received in said receptacle of said receiver portion and a distal portion extending from said head for engaging a bony structure of the spinal column;a retaining clip engageable to said coupling member in a distally oriented opening of said receptacle to pivotally capture said head in said receptacle;2005294535 03 Jan 2012 a crown positioned about said head of said anchor in said receptacle, said crown including at least one seat portion extending through said proximally opening window for positioning in contact with said implant;and a locking member engageable to said post in contact with said implant to secure said implant to said coupling member in contact with said seat portion.

    The assembly of any one of claims 21 to 29 and 31, or the system of any one of claims 30 to 32, wherein said seat or seating portion of said crown includes a number of axially extending arms, and said at least one window of said coupling member includes a number of axially extending windows, each of said number of arms being positioned through a respective one of said number of windows.

    A spinal surgical method, comprising:engaging an anchor member of an anchor assembly to at least one vertebra, the anchor assembly including a coupling member with a receiver portion defining a receptacle for receiving the anchor member, a post extending proximally from the receiver portion, and a crown in the receptacle adjacent a proximal end of the anchor 2005294535 03 Jan 2012 member, the crown including at least one proximally extending seat portion extending from the receptacle to a location outside the receptacle;pivoting the coupling member relative to the engaged anchor member to orient the post in a desired position;advancing a member along the post to a location adjacent the anchor member;and engaging the member against the at least one seat portion of the crown to engage the crown to the proximal end of the anchor in the receptacle.
